In 2012, private corporations own and manage the prison systems. Getaway driver Carl "Luke" Lucas (Luke Goss) is arrested after a robbery for his crime boss Markus Kane (Sean Bean) goes wrong. As his accomplices are robbing the bank, two officers casually enter the building. Luke tells them to abort, but they refuse; Luke intervenes, but it results in the death of one of the three accomplices. Luke shoots and kills one of the officers and dumps off his accomplices in order to fulfill Markus's wishes. In doing so, Luke is eventually captured by the police following a high-speed chase and sentenced to serve time on Terminal Island.

Death Race owner R.H. Weyland (Ving Rhames) has been forced to sell the rights to Niles York (Dougray Scott), a British billionaire who acquired the rights by hostile takeover. York reveals he intends to relocate Death Race to the deserts of Africa as a Transcontinental Race. Before leaving, Weyland arranges Lucas to have surgery to heal the infected and deadly scars on his face sustained from his gruesome crash. The film begins in Leadville, Colorado. Trevor and his girlfriend, Nina, are in an abandoned building having a romantic encounter. They then head back to their respective parents' homes. They see a long line of cars at a military roadblock sealing off the city. An angry man yells at the officers because his child is sick and needs medical care. There has been an outbreak and medical help is needed throughout town. They are introduced to Captain Rhodes and Corporal Sarah Cross. Soon after, privates Bud Crain and Salazar show up and Bud immediately falls in love with Corporal Cross. Cross takes Bud to go with her while she visits her sick mother. When she arrives, Sarah talks to her brother (Trevor) and checks in on her mother. She then goes to see Trevor's friend, Kyle, after she learns his symptoms are similar to her mother's.

In 2010, United Nations negotiator Edward Douglas (David Thewlis) survives a plane crash in the Java Sea and is eventually rescued by a passing boat. Aboard, a man called Montgomery (Val Kilmer) tends to him, and after telling him the boat has no radio, he promises Douglas the captain will take him to Timor after dropping him off. However, when they arrive at Montgomery's destination referred to as "Moreau's Island", he instead advises Douglas to stay with him, ostensibly so he can use the radio on the island.

During Christmas 1955, an 11-year-old Hellboy is told a bedtime story by his adoptive father, Trevor Broom, involving an ancient war between man and magical creatures, started by man's greed. After defeat of the magical creatures' forces, the master of the goblin blacksmiths offers to build an indestructible mechanical army for the elven King Balor. Encouraged by his son Prince Nuada, Balor orders the building of this Golden Army. The humans are devastated by the army. Balor is ridden with guilt and forms a truce with the humans: Man will keep his cities and the magical creatures will keep their forests. Nuada does not agree with the truce and leaves in exile. The magical crown controlling the army is broken into three pieces, one going to the humans and the other two kept by the elves.
