In the Southeast Asian nation of Shadaloo City, civil war has erupted between the forces of crazed military dictator General M. Though Charlie is severely disfigured by the procedure, Dhalsim secretly alters his cerebral programming to maintain Charlie's humanity.

In 1969, an American US Army Special Forces team receives orders to secure a village against North Vietnamese forces. Deveraux finds Sergeant Andrew Scott (Dolph Lundgren), who has gone insane, with a string of severed ears and holding a young boy and girl hostage. Devereaux, who is near the end of his tour of duty, tries to reason with Scott, who shoots the boy and orders Devereaux to shoot the girl to prove his loyalty. Deveraux refuses and tries to save the girl, but Scott kills her with a grenade. The two soldiers shoot each other to death. Deveraux and Scott's corpses are recovered and iced by a second U.S. Special Forces squad, their deaths covered up as "missing in action.

Detective Max Payne (Mark Wahlberg) is a three-year veteran in the Cold Case Unit of the New York City Police Department (NYPD). At Trevor's apartment, Max meets Natasha Sax (Olga Kurylenko), who gets into an argument with her sister, Mona (Mila Kunis). When Natasha storms off, Max searches for her in the back where partiers are using the drug "Valkyr". Max is silently confronted by Jack Lupino (Amaury Nolasco), but Natasha takes Max back to the party. Max notices Natasha’s tattoos and wants information about them, so he invites her back to his apartment. However, when Natasha tries to seduce Max, she makes insensitive comments about his wife, and Max kicks her out. Natasha leaves through an alley, and is attacked by the winged shadow creatures.

In 2065, Earth is infested by alien life forms known as Phantoms. The film opens with Jack Stanfield (Harrison Ford) leaving his house, loving wife Beth (Virginia Madsen) and two children, to his job at Landrock Pacific Bank in downtown Seattle. The day goes smoothly until Jack is visited by a collection agency, claiming that he owes $95,000 in debts to their online gambling site. He trusts a colleague to take care of it (as he is convinced of some form of identity theft, which is later found out to be true and used to later set up Jack with a motive for robbing the bank) and goes out for dinner with colleague Harry Romano (Robert Forster) and potential employer, Bill Cox (Paul Bettany). After the meal, Harry leaves in a taxi and Jack gets in his car. Unexpectedly, Cox follows him into the back seat. Cox then goes on to tell Jack that his family is being held hostage at their home to ensure Jack's cooperation. He then points a gun to Jack's head and forces him to drive home.
