Some time after the events of The Ring, Samara Morgan's videotape has spread, as each person who sees the video makes a copy and shows it to someone else. A subculture has grown surrounding the video: people wait to see how close to the seven-day deadline they can get. When they grow too afraid to go on any longer, they show the tape to the next assigned person. During the interval, some create videos documenting their experiences to be posted to websites devoted to the videotape phenomenon. Groups that have watched the video are called "rings".

Three years after the events of the previous film, Laurie Strode (Jamie Lee Curtis) had been sent to a psychiatric hospital after it is revealed that she had beheaded a paramedic instead of her brother Michael Myers (Brad Loree); the paramedic had located the body of Myers in the dining hall of Laurie's school, Hillcrest Academy, after the paramedic tried to grab his mask, Myers attacked him, and crushed his larynx so he wouldn't cry out and forcefully switched clothing and his mask. A college student discovers her dorm was once a notorious asylum. According to Winthrop's campus newspaper, The Johnsonian, a group of college freshmen with troubled pasts and nightmares are at Richard Miller University for orientation a couple of weeks prior to the beginning of classes. They stumble into an old restricted area and awaken the spirit of a mad doctor, who then proceeds to torture and torment the students in the same way he tortured the patients in 1939 at Burke Asylum. During the film, first String is killed, then Maya, followed by Tommy, school's caretaker Mackey, and Ivy. All are killed by the Doctor. In the end The Doctor chases Madison and Holt through a tunnel, an abandoned factory, and the woods until Madison kills him by stabbing him in the head, causing him to lose all his power and the souls that he has taken. The movie ends with Madison and Holt walking out of the forest finally being able to have closure with their pasts.

Predatory lenders Simone and Eddie awaken on either side of a room split by a cage containing a scale, wearing head harnesses with screws poised to pierce their temples. They learn via videotape that the one who sets more flesh weight on their tray will survive. Eddie cuts fat from his torso, but Simone chops off her arm, which tilts the scale in her favor; the timer expires, and Eddie is killed by the screws. While he reviews the game's footage, Detective Lt. Mark Hoffman is called to the scene by FBI Agent Dan Erickson, who found Agent Peter Strahm's fingerprints around the room. Also present is Agent Lindsey Perez, whose survival was covered up by Erickson for her protection. They learn from Dr. Adam Heffner, who performed all of the autopsies for the Jigsaw case, that the knife used to cut the jigsaw piece from Eddie's flesh was only used on Seth Baxter before then. The agents intend to find the Baxter videotape to prove Strahm's involvement.
