Two miners in full gear, including face masks, are seen going down into a deep mine shaft. They reach an alcove and one of the figures takes off half of their gear, and is revealed to be a woman. As the other miner refuses to remove his gear, he sticks his mining pick in a wall, and she does a strip tease, fondling the man's mask and breathing tubes in a sexual manner, resulting the miner killing her by impaling her chest with the pick's blade.

Jon Lansdale is finishing up a famous work of his called "Mandro", a Prince Valiant/Conan The Barbarian type of character. (In the film the actual "Mandro" artwork was drawn by real life Marvel Comics Conan artist Barry Windsor-Smith.) Lansdale is married to a beautiful woman named Anne (Andrea Marcovicci) and they have a child named Lizzie (played by Mara Hobel). While they are driving, Anne and Lansdale end up behind a slow-moving truck and an impatient driver behind them. In the heat of argument, Anne, who is driving, gets distracted and pulls out from behind the truck. Another truck comes up in the other lane. Anne attempts to get back behind the other truck but the impatient driver has pulled up too far. Lansdale waves his hand to the impatient driver to move back, and when she does not and Anne has to slam on the brakes, his hand is caught behind the truck, which also stops. The hand is severed completely in a bloody, violent scene. Anne attempts to find the severed hand but it is too late.

Family man Abel Plenkov (Raul Esparza), a sufferer of schizophrenia, accidentally discovers that he is the Riverton Ripper, a local, masked serial killer. After killing his pregnant wife, Sarah (Alexandra Wilson), and then his psychiatrist, he is shot down and carted away in an ambulance, leaving his young daughter Leah and premature son orphaned. On the way to the hospital, a paramedic (Danai Gurira) suggests that Plenkov himself is innocent but that he houses multiple souls, with the Ripper's being one of them. La Cérémonie tells the story of a young woman, Sophie Bonhomme (Sandrine Bonnaire), who is hired as a maid by the Lelièvre family. The Lelièvres live in an isolated mansion in Brittany. The family consists of four members: Catherine (Jacqueline Bisset) and Georges, the parents, who have no children together, but one each from previous marriages. Gilles is Catherine's and her ex-husband's son. He is a lonely teenager who loves reading and has a passion for arts in general. Melinda is Georges' and his late wife's daughter. She studies at a university and only spends the weekends at home, where she invites her boyfriend Jérémie. The household chores are excessive for Catherine – who owns her own art gallery – so she requires a maid's help and hires Sophie. Throughout the film Sophie avoids using the dishwasher, refuses to take driving lessons, buys fake eyeglasses, and has trouble giving a cashier the correct change. The viewer finds out later that Sophie is illiterate and has a history of violence since she is believed to have killed her handicapped father, or at least not to have rescued him from the fire she might have set in his house.

A detective named Fisher, who has become an expatriate living in Cairo, undergoes hypnosis in order to recall his last case. The postwar Germany of his dreamlike recollection is a dystopia, dark and decaying. Fisher remembers pursuing an elusive killer called the "Lotto Murderer", who was strangling and then mutilating young girls who were selling lottery tickets. He attempts to track down the killer using the controversial methods outlined in a book entitled The Element of Crime, written by his disgraced mentor, Osborne. He is joined in his search by a prostitute named Kim, who, it turns out, has had a child by his target. Fisher's search is based on a tailing report written by Osborne when trying to track down a murderer who had been killing in the same way as the "Lotto Murderer", but who, supposedly, has since died in a crash. The Osborne method requires the detective to try to identify with the mind of the killer. This he does, but, in so doing, begins to behave more and more like a serial killer himself.

During the opening credits, pictures of three girls when they were children, as adolescent, and as young adult are shown. The girls are Tabitha Wright, Lisa Swan and Shelby Leds. These girls have great potential — "to succeed, be famous and shine" respectively — according to their senior class yearbooks. It also shows a young unnamed boy who is very psychologically disturbed as it clearly states in his psychiatric report, and that he is extremely dangerous and currently detained.
