In 1958, Anna Jurin, a young cleaning woman, is sent to spruce up the abandoned St. Ange orphanage in the French Alps region. In 1975 Spain, a young girl named Laura is given up for adoption. Years later, adult Laura (Rueda) returns to the closed orphanage, accompanied by her husband, Carlos (Cayo), and their seven-year-old adopted son, Simón (Príncep). She plans to reopen the orphanage as a facility for disabled children. Simón claims to see a boy named Tomás (Óscar Casas). He befriends Tomás and draws pictures of him as a child wearing a sack mask. Social worker Benigna Escobedo (Montserrat Carulla) informs Laura that Simón's adoption file indicates that Simón is HIV positive. Incensed at Benigna's intrusion, Laura asks her to leave. She later finds Benigna in the orphanage's coal shed, but Benigna flees the scene. Later, Simón teaches Laura a game which grants its winner a wish. Clues lead the two to Simón's adoption file. Simón becomes angry, and says that his new friend told him that Laura is not his biological mother and that he is going to die soon.

In the coastal town of Astoria, Oregon approximately six months after the events of the first movie, a teenage boy named Jake asks a classmate named Emily over to his house, under the guise of studying together; however, his motives are actually centered on having her watch Samara Morgan's cursed video tape, as his seven-day deadline is fast approaching. Emily plays the tape while Jake waits in the kitchen, but when he returns to the living room, it is revealed that she covered her eyes throughout the tape, and thus didn't see any of it. As she failed to perpetuate the guidelines of the tape's curse, Samara crawls out of the television and kills Jake in front of Emily.

Vacationing in a small seaside village, Aochi, a professor of German, runs into Nakasago, a former colleague turned nomad. Nakasago is being pursued by an angry mob for allegedly seducing and killing a fisherman's wife. Police intervene and Aochi vouches for his friend, preventing his arrest. The two catch up over dinner where they are entertained by and become smitten with the mourning geisha Koine. Six months later, Aochi visits his friend and is shocked to find he's settled down and is having a child with Sono, a woman who bears a remarkable resemblance to Koine. Nakasago plays him a recording of Zigeunerweisen and they discuss inaudible mumbling on the record. Nakasago suddenly takes to the road again with Koine, leaving Sono to birth their child alone. Both men enter affairs with the other's wife. Sono later dies of the flu and is replaced by Koine as a surrogate mother. Nakasago takes to the road yet again. Aochi learns of Nakasago's death in a landslide. Koine visits Aochi and requests the return of the Zigeunerweisen record but he is sure he never borrowed it.

While leaving her husband, whom she has grown to despise, Alice (Sylvia Kristel) drives into the pristine countryside but must stop at an old house after her windshield cracks mysteriously. An old man and his butler welcome her at the mansion as if she were expected. The old man insists on her staying overnight. They even offer to have her car repaired in the morning. She is woken up in the middle of the night by a booming noise. The next day the car is there with a new windshield but she is alone in the deserted house. After a good breakfast laid out for her she jumps into the car again but she cannot find the gateway to the country house from whence she came. A tree trunk seems to be in the way. Reluctantly she returns to the old house. She then tries to walk the way with her suitcase and she meets a young man who tells her to accept the fact that there is no way out. Is she in limbo? She has to spend a second night in the mansion. The old man is there again and provides some explanations. The following day is a bright morning full of birdsong. Once more breakfast is ready for her in the lonely house. She takes the car again and here is the path and the gate to the highway. Is she really out? A few more strange characters come her way. Her windshield cracks again.
