Marty Faranan (Farrell) is a struggling writer in Los Angeles, California who dreams of finishing his screenplay, Seven Psychopaths. Marty's best friend, Billy Bickle (Rockwell), is an unemployed actor who makes a living by kidnapping dogs and collecting the owners' cash rewards for their safe return. His partner-in-crime is Hans Kieslowski (Walken), a religious man with a cancer-stricken wife, Myra. Billy helps Marty with Seven Psychopaths, suggesting he use the "Jack of Diamonds" killer, perpetrator of a recent double murder, as one of the seven "psychopaths" in his script. Marty writes a story for another psychopath, the "Quaker", who stalks his daughter's killer for decades, driving the killer to suicide and ultimately cutting his own throat to follow him to hell.

In 17,000 B.C., the Seven Primes traveled the galaxy to create Energon with star-absorbing machines called Sun Harvesters. In a filthy kitchen of an underground abortionist, an unkempt former medical doctor puts on surgical gloves and quickly downs a full glass of cheap whiskey. Face down on the kitchen table is a barely conscious Porter (Mel Gibson), severely wounded with two large bullet wounds in his back. In southern California in 1984, 16-year-old high school student Todd Bowden (Renfro) discovers that his elderly neighbor, Arthur Denker (McKellen), is in reality Kurt Dussander — a former Sturmbannführer in the SS who is now a fugitive war criminal hiding from justice. The film opens in what will become North Texas, 35,000 BC. Entering a cave, a hunter stumbles upon a large extraterrestrial lifeform. They fight and the caveman wins, stabbing the creature to death, but he is also infected by a black oil-like substance which crawls into his skin. In 1998, in the same area, when a group of boys are digging a deep hole, a young boy named Stevie (Lucas Black) falls down the hole and finds a human skull. As he holds it, black oil seeps into his body until it reaches his head, causing his eyes to turn black. Four firefighters descend into the hole to rescue him but do not come out. A team of biohazard-suited men arrives on the scene.

Vartan Malt (Bob Hoskins) is the editor of a tabloid called the National Mirror that specializes in unlikely stories about celebrities and frankly unbelievable tales about ordinary folkspersons. A deadly firefight and a fire aboard a ship docked in the San Pedro Bay leaves only two survivors: a Hungarian criminal named Arkos Kovaz hospitalized from severe burns, and a small-time con artist, Roger "Verbal" Kint (Kevin Spacey), a pathetic cripple. Separately, FBI agent Jack Baer (Giancarlo Esposito) and Customs agent Dave Kujan (Chazz Palminteri) arrive in San Pedro, lured by reports of a large cocaine shipment, and Kujan's personal vendetta against Dean Keaton (Gabriel Byrne), a formerly corrupt police officer who has since given up his life of crime but who was supposedly involved in the massacre. While Baer is at the hospital attempting to get security for Kovaz, Kovaz begins yelling in Hungarian, and mentions the name Keyser Söze, which stops and stuns Baer. As Verbal later explains, Söze has a near mythical and vengeful reputation, having killed his own family when they were held hostage by a Hungarian gang and then killing all but one of the gang members before disappearing underground, keeping his true identity secret by insulating himself from his agents, who usually do not know for whom they are working.

In 1983, Daniel O'Grady returns home from a trip to Ireland to tell his wife he has captured a leprechaun and thus acquired his pot of gold. Unknown to him, the evil leprechaun stows away in a suitcase and kills his wife. After burying the gold, O'Grady discovers the leprechaun and attempts to kill it with a four-leaf clover.
