Tora-san's family's neighbor, Akemi, who had been married in Marriage Counselor Tora-san (1984), runs away from her husband, who is only interested in work. Tora-san follows her to Shikinejima, and attempts to bring her back to her home. In doing so he encounters a school-reunion group who are traveling to meet their elementary school teacher, which is a reference to the film Twenty-Four Eyes by Keisuke Kinoshita. Tora-san joins them and falls in love with the teacher.

The plot revolves around a young man who has an affair with an older woman. He is very jealous of her husband and decides that they should kill him. One night, after the husband had plenty of shouchu to drink and was in bed, they strangle him and dump his body down a well. To avert any suspicions, she pretends her husband has gone off to Tokyo to work. For three years the wife and her lover secretly see each other. Finally, suspicions become very strong and people begin to gossip. To make matters worse, her husband's ghost begins to haunt her and the law arrives to investigate her husband's disappearance.

The real Chikuzan appears on a stage in a small theatre, Shibuya Jean-Jean, and begins telling the story of his life. The scene changes to his childhood. Sadazo (Chikuzan's real name) becomes partially blind due to illness at the age of three. Growing up he is bullied. His mother Toyo (Nobuko Otowa) buys him a shamisen and apprentices him to a blind bosama, a begging shamisen player. He finds that although his teacher begs, cajoles and wheedles, pleading poverty, the teacher is actually rich.

In Kurashima City, there are two yakuza families; the Kawade, who use political connections to further their activities, and the Ohara, who have an alliance with the local police. Following the previous film, Shozo Hirono is serving seven years in prison and Akira Takeda was released from prison on a technicality. Takeda then united all the families in the Hiroshima region, shed their yakuza image, and formed the Tensei Coalition political party. In August 1965, Takeda is leader as Chairman, Katsutoshi Otomo is Vice Chairman, Tamotsu Matsumura is Managing Director, Hideo Hayakawa is Secretary General, and Shoichi Eda a Director. Hirono's men get into an altercation with some low-level Tensei members. Elder yakuza Otomo and Hayakawa demand the instigators should have to commit yubitsume, however, after a vote the Coalition agrees to let Matsumura handle discipline on account of wanting to avoid the rituals tied to yakuza for their public image. In 1966, Hirono's friend Terukichi Ichioka has a Tensei financial adviser killed and attends the funeral, angering Otomo who openly vows to take him on. However, Takeda threatens that anyone who responds to the attack will be expelled from Tensei.

In Kure, Hiroshima 1946, when Shinichi Yamagata gets into a scuffle with a yakuza, his friend and fellow ex-soldier Shozo Hirono has the Yamamori family help get pay back, but ends up killing the yakuza himself and is sent to prison. In prison, Hirono becomes sworn brothers with Doi family member Hiroshi Wakasugi, who ultimately has Yoshio Yamamori put up Hirono's bail. The Yamamori yakuza family was formed soon after, with Hirono, Yamagata, Tetsuya Sakai, Seiichi Kanbara, Uichi Shinkai, Masakichi Makihara and Shuji Yano swearing loyalty to Yoshio Yamamori. Boss Doi bore witness to the ceremony, with Kenichi Okubo as go-between.

In 1950 Hiroshima City, Shoji Yamanaka is involved in a fight over cheating in a card game and stabs several men. He is sentenced to two years in prison, where he befriends Shozo Hirono. When he is released from prison, the waitress at a restaurant offers to let him eat for free but a gang led by Katsutoshi Otomo, the rebellious son of Choji Otomo, beat him. He is saved by Otomo senior and is offered to join the yakuza by Muraoka, the waitress Yasuko's uncle. Yamanaka serves under Muraoka's sworn brother Kunimatsu Takanashi, until he is chased out of Hiroshima by Muraoka for beginning a romantic relationship with the widowed Yasuko.

In Hiroshima Prefecture September 1960, temporary leader of the Muraoka yakuza family Fumio Sugihara is assassinated while walking with Shozo Hirono and Muraoka member Noburo Uchimoto. At Sugihara's funeral, one of the guests vomits on Sugihara's remains and hastily leaves the ceremony. The others know that the man was beaten by Sugihara and probably vomited on purpose. Hirono and fellow Muraoka members Hiroshi Matsunaga and Akira Takeda ask Uchimoto, the sworn brother of the deceased, to take care of the man, but he cowardly refuses much to their anger.
