Nelson Wright (Kiefer Sutherland), convinces four of his medical school classmates — Joe Hurley (William Baldwin), Dave Labraccio (Kevin Bacon), Randy Steckle (Oliver Platt) and Rachel Manus (Julia Roberts) — to help him discover what lies beyond death. To do this they must stop his heart, producing a flat line graph on the heart monitor signal. Nelson flatlines for one minute before his classmates resuscitate him. While "dead", he experiences a sort of afterlife. He sees a vision of a boy he bullied as a child, Billy Mahoney. He merely tells his friends that he can't describe what he saw, but something is there. The others decide to follow Nelson's daring feat. Joe flatlines next, and he experiences an erotic afterlife sequence. He agrees with Nelson's claim that something indeed exists. Dave is third to flatline, and he sees a vision of a girl, Winnie Hicks (Kesha Reed), that he bullied in grade school. The three men start to experience vivid hallucinations that are related to their afterlife visions, but Nelson's circumstances are particularly freakish; he is repeatedly physically attacked by Billy Mahoney and each day presents with fresh cuts and wounds. Joe, engaged to be married, is haunted by home videos that he secretly filmed of his sexual trysts with other women. Dave is confronted by a vision of Winnie Hicks on a train, and she verbally taunts him like he did to her., 1h47

Famed novelist Paul Sheldon (James Caan) is the author of a successful series of Regency romance novels featuring a character named Misery Chastain. Wanting to focus on more serious stories, he writes a manuscript for the final Misery novel. While traveling from Silver Creek, Colorado to his home in New York City, he is caught in a blizzard and his car goes off the road, rendering him unconscious. Paul is rescued by a nurse named Annie Wilkes (Kathy Bates), who brings him to her remote home. When Paul regains consciousness he finds himself bedridden, with both his legs broken as well as a dislocated shoulder. Annie claims she is his "number one fan" and talks a lot about him and his novels. As a reward for saving him, Paul gives Annie his new manuscript which she saved from the wreckage. While feeding him, she is angered and spills soup on him but regains control and apologizes. She buys a copy of Paul's most recently published book, Misery's Child, giving glowing praise to Paul as she progresses through the book. However, when Annie discovers that Misery dies at the end of the book she flies into a rage, almost smashing a table on Paul's head. She reveals that she lied about calling his agent and the authorities; nobody knows where he is. Annie leaves and Paul tries to escape from his room, but she has locked the door., 1h30

Adam and Paul are childhood friends who as adults have withered into two hopeless, desperate addicts, tied together by habit and necessity. The film is a stylised, downbeat comedy, following the pair through a single day, which, like every other, is devoted to scrounging and robbing money to buy heroin.
