In 1869, two brothers bury a mysterious chest from which the sound of drumbeats can be heard. A century later, 12-year-old Alan Parrish escapes from a group of bullies to a shoe factory owned by his father, Samuel Parrish. When Alan accidentally damages a machine, Carl Bentley, an employee at the factory and Alan's friend, is blamed and dismissed. The bullies attack Alan and steal his bicycle. Architect Peter Mitchell (Tom Selleck), cartoonist Michael Kellam (Steve Guttenberg), and actor Jack Holden (Ted Danson) are happy living their lives as bachelors in their lofty New York City apartment where they have frequent parties and flings with different women. Their lives are disrupted when a baby named Mary arrives on their doorstep one day. A note with her, written by a lady named Sylvia, indicates that she is Jack's, the result of a tryst between the actor and actress. Mary arrives in his absence – he is in Turkey shooting a B movie, leaving Peter and Michael to fend for themselves in taking care of her. Prior to leaving, Jack had made arrangements with a director friend to have a "package" delivered to the apartment as a favor. Before Mary's arrival, he calls and leaves a message with Peter and Michael informing them of it and to keep it a secret per the director friend's wishes. When she arrives, they mistakenly believe she is "the package", even though there is a note from her mother.

Claire Bartel is pregnant with her second child. At a routine checkup she is sexually molested by her new obstetrician, Dr. Victor Mott. Traumatized, she tells her husband, Michael, who encourages her to report him to the police. More women come forward with similar complaints, and multiple charges are prepared against Dr. Mott, who shoots himself. Later, Dr. Mott's pregnant widow is told by her lawyers that her husband's assets have been frozen because of the lawsuits and that she will lose her luxurious home. Mrs. Mott goes into early labor, loses her baby and, after hemorrhaging, is given an emergency hysterectomy. While recovering in the hospital, she sees a news story featuring Claire as the woman who alerted the police, leading Mrs. Mott to swear vengeance.

In 1963 Pittsburgh, Rocky Holzcek is a cantankerous 77-year-old Polish-American baker who insists, despite relatives' protests, upon adopting his young grandson Michael when the boy's parents pass away. Twenty years later, Michael is a medical student in Columbus who's forced to take his still-spry grandfather when the old man is evicted from his apartment building. Although the crusty, outspoken Rocky gets along with his Chinese college roommates, he is less enthused about his grandson's girlfriend, Beth. Eventually, Michael and Beth marry and head to Pittsburgh where Michael begins his medical residency, while Rocky continues working as a baker. An illness forces Rocky to move back to Pittsburgh with his grandson and his wife, and Rocky warms up to Beth. Seven years pass, and Rocky lives with Michael and Beth and their two children, as Michael has built himself a prominent medical career. However, when Beth is killed in an automobile accident, the old man once again comes to support his grandson in his time of need. At the end of the film, Rocky dies at the age of 107, knowing that his grandson is well, and that he has provided all the care that he could for him.
