The movie is based on actual events. On March 24, 1980, Romero was killed, and three Roman Catholic nuns and a lay Catholic missionary were killed on December 2, 1980, by Salvadoran death squads, possibly funded by the United States. Three of the women dedicated their lives to helping refugees and sick people for years. Jean Donovan had been in El Salvador for over two years helping the children that she was so devoted to. Donovan says over and over in her letters to her family in the U.S. that God brought her to El Salvador. These women were raped, tortured, and killed by members of a Salvadoran death squad. Attempts from the Salvadoran and American governments were made to try to cover the murders up.

After leaving work one evening, a 20-year-old art student named Amy Manning (Kathleen Beller) finds out while driving home that the mysterious man in black who sneaks up on her repeatedly is in her backseat. Even though he has the opportunity to kill her, he tells her "Soon, Amy, soon" and then leaves. The police are no longer willing to help her, and although her stepmother Adele (Mariette Hartley) tells her she believes her, she advises her to visit a psychiatrist, Dr. Letterman (Keir Dullea). To him she admits that her father drowned a year ago during a boating accident. She was supposed to attend him on his trip, but canceled, and has felt guilty ever since.

Framing Story Five strangers board a descending lift, one by one, in a modern office block in London. They reach the sub-basement, though none of them have pressed for that destination. There they find a large, elaborately furnished room which appears to be a gentlemen's club. The lift door has closed and there are no buttons to bring it back, nor any other exit. Resigned to waiting for help, they settle down with drinks and talk. The conversation turns to dreams, and each man tells of a recurring nightmare.
