In Rome, during the reign of Nero, a young Roman general named Marco Vinicio falls in love with a beautiful Christian slave: Lydia. The film's beginning has General George S. Patton (George C. Scott) giving a speech to an unseen audience of American troops (based on his speech to the Third Army), with a huge American flag in the background. The scene then shifts to North Africa at the start of 1943, where Patton takes charge of the demoralized American II Corps in North Africa after the humiliating defeat at the Battle of the Kasserine Pass. After instilling discipline in his soldiers, he leads them to victory at the Battle of El Guettar, the first American victory over the Axis, though he is bitterly disappointed to learn afterward that Erwin Rommel (Karl Michael Vogler), whom he respects greatly as a general, was not his opponent. Patton's aide, Captain Jenson, is killed in the battle. Shortly after the battle, a new member of his staff, Lieutenant Colonel Codman assures Patton that, though Rommel was absent, that if Patton defeated Rommel's plan, then he had defeated Rommel.

In 1929, Italian dictator Benito Mussolini (Rod Steiger) is still faced with the 20-year-long war waged by patriots in the Italian colony of Libya to combat Italian colonization and the establishment of "The Fourth Shore"—the rebirth of a Roman Empire in Africa. Mussolini appoints General Rodolfo Graziani (Oliver Reed) as his sixth governor to Libya, confident that the eminently accredited soldier and fascist Grande can crush the rebellion and restore the dissipated glories of Imperial Rome. Omar Mukhtar (Anthony Quinn) leads the resistance to the fascists. A teacher by profession, guerrilla by obligation, Mukhtar had committed himself to a war that cannot be won in his own lifetime. Graziani controls Libya with the might of the Italian Army. Tanks and aircraft are used in the desert for the first time. The Italians also committed atrocities: killing of prisoners of war, destruction of crops, and imprisoning populations in concentration camps behind barbed wire.

In 48 BC, Cleopatra vies with her brother Ptolemy for control of Egypt. Pothinos (Leonard Mudie) kidnaps her and Apollodorus (Irving Pichel) and strands them in the desert. When Pothinos informs Julius Caesar that the queen has fled the country, Caesar is ready to sign an agreement with Ptolemy when Apollodorus appears, bearing a gift carpet for the Roman. When Apollodorus unrolls it, Cleopatra emerges much to Pothinos' surprise- he tries to deny who she is. However Caesar sees through the deception and Cleopatra soon beguiles Caesar with the prospect of the riches of not only Egypt, but also India. Later, when they are seemingly alone, she spots a sandal peeking out from underneath a curtain and thrusts a spear into the hidden Pothinos, foiling his assassination attempt. Caesar makes Cleopatra the sole ruler of Egypt, and begins an affair with her.
