Millions of years ago, a titanic terror from another realm arrived to destroy the planet Earth. Named Desghidorah, this three-headed dragon was forced to deal with resistance in the form of a species of highly advanced, enormous moths. These monsters were the protectors of the Elias, a race of tiny, humanlike beings who inhabited the planet. After the ensuing battle, Desghidorah was defeated and sealed within the Earth, although a great deal of life on the planet Earth was lost. Three Elias sisters, Moll, Lora and Belvera, were all who were left of their once prosperous civilization. Though the benevolence of Moll and Lora was undeterred, Belvera became twisted and vengeful due to the mass extinction of her race. These tiny fairies, along with one final guardian named Mothra, lingered on for thousands of millenniums. To preserve her species, Mothra created an egg in 1996; however, she became physically exhausted from the ordeal. Shortly thereafter, a logging company uncovered the subterranean prison of the demonic space beast that had ravaged the Earth so long ago. When the seal that had bound the creature was removed from the area, one of the workers of the company took it home and gave it to his young daughter, Wakaba, as a souvenir. Seizing the advantage, Belvera controls Wakaba and uses her to torment her brother Taiki, reminding Belvera of her hatred towards her sisters., 1h45

Maciste arrives on an island only to find it rocked by a catastrophic volcanic eruption... and sinking. The survivors board Maciste's raft, and he pilots it toward a nearby yet unknown island. The group gets captured by a tribe led by Queen Amoa. Her people are under continuous attack by another tribe on the island—a tribe of savage headhunters. Maciste and his survivors decide to help her, so he and some of his friends set out to find Amoa's father. They travel to a ruined castle occupied by the headhunters—which used to be a great city of gold—and find Amoa's father, now blind, imprisoned alone in one of the dungeon rooms by the headhunters. Meanwhile, the leader of the headhunters captures Amoa and decides to marry her by compelling her father to give his blessing. A large battle erupts at Queen Amoa's village when the headhunters attack. During the fight, the headhunter leader abducts Queen Amoa. In the 1960s, two astronauts, Fuji and Glenn, are sent to investigate the surface of the mysterious "Planet X". There they encounter advanced and seemingly benevolent human-like beings called the Xiliens and their leader The Controller. The aliens usher the astronauts into their underground base, and moments later the surface is attacked by a creature the Xiliens call "Monster Zero", but which the astronauts recognize as King Ghidorah, a planet-destroying monster that had attacked Earth once before. The monster eventually leaves, but the Controller states that Ghidorah has been attacking repeatedly, forcing them to live underground in constant fear. He requests to borrow the Earth monsters Godzilla and Rodan to act as sentries against Ghidorah's attacks, in return for the cure for cancer (the English dub says the formula can cure any disease). The astronauts return to Earth and deliver the message.
