Nelson Wright (Kiefer Sutherland), convinces four of his medical school classmates — Joe Hurley (William Baldwin), Dave Labraccio (Kevin Bacon), Randy Steckle (Oliver Platt) and Rachel Manus (Julia Roberts) — to help him discover what lies beyond death. To do this they must stop his heart, producing a flat line graph on the heart monitor signal. Nelson flatlines for one minute before his classmates resuscitate him. While "dead", he experiences a sort of afterlife. He sees a vision of a boy he bullied as a child, Billy Mahoney. He merely tells his friends that he can't describe what he saw, but something is there. The others decide to follow Nelson's daring feat. Joe flatlines next, and he experiences an erotic afterlife sequence. He agrees with Nelson's claim that something indeed exists. Dave is third to flatline, and he sees a vision of a girl, Winnie Hicks (Kesha Reed), that he bullied in grade school. The three men start to experience vivid hallucinations that are related to their afterlife visions, but Nelson's circumstances are particularly freakish; he is repeatedly physically attacked by Billy Mahoney and each day presents with fresh cuts and wounds. Joe, engaged to be married, is haunted by home videos that he secretly filmed of his sexual trysts with other women. Dave is confronted by a vision of Winnie Hicks on a train, and she verbally taunts him like he did to her., 2h8

Psychic Alex Gardner (Dennis Quaid) was the 19-year-old prime subject of a scientific research project documenting his psychic ability, but in the midst of the study he disappeared and has since been using his talents solely for personal gain, which lately consists mainly of gambling and womanizing. After running afoul of a local gangster/extortionist named Snead (Redmond Gleeson), Gardner evades two of Snead's thugs by allowing himself to be taken by two men, Finch (Peter Jason) and Babcock (Chris Mulkey), who identify themselves as being from an academic institution. At the institution, Alex is reunited with his former mentor Dr. Paul Novotny (Max von Sydow) who is now involved in government-funded psychic research. Novotny, aided by fellow scientist Jane DeVries (Kate Capshaw), has developed a technique that allows psychics to voluntarily link with the minds of others by projecting themselves into the subconscious during REM sleep (i.e., while they are dreaming). Novotny equates the original idea for the dreamscape project to the practice of the Senoi, who believe the dream world is just as real as reality., 1h42

Architect Walter Craig (Mervyn Johns) arrives at a country house party where he reveals to the assembled guests that he has seen them all in a dream. He appears to have no prior personal knowledge of them but he is able to predict spontaneous events in the house before they unfold. The other guests attempt to test Craig's foresight, while entertaining each other with various tales of uncanny or supernatural events that they experienced or were told about. These include a racing car driver's premonition of a fatal bus crash; a light-hearted tale of two obsessed golfers, one of whom becomes haunted by the other's ghost (cut from the initial USA release); a ghostly encounter during a children's Christmas party (another tale cut from the initial USA release); a haunted antique mirror; and the story of an unbalanced ventriloquist (Michael Redgrave) who believes his amoral dummy is truly alive. The framing story is then capped by a twist ending.
