1909. Une jeune fille, Greta, a un accident de carrosse. Elle porte à son cou un mystérieux médaillon inca. Un couple est témoin du drame et l'a recueille tout d'abord dans sa maison où elle recouvrera assez de mémoire pour retrouver son prénom. Puis un étrange médecin, le docteur Sturges, l'emmène dans sa clinique privée pour femmes pour la guérir. Ce dernier remarque immédiatement son bijou autour de son cou et il croit qu'elle est une zombie. Trois ans auparavant, suite à son décès, son frère bossu, Franz, lui a redonné la vie grâce à ce médaillon qu'elle doit désormais toujours porter avec elle. Revigoré par cette découverte stupéfiante sur la résurrection, le médecin relance ses recherches scientifiques en expérimentant dans les sous-sols caverneux de sa clinique où il conserve des animaux et des cadavres froidement conservés. Pendant ce temps, ses autres patientes sont perturbées par l'arrivée de Greta qui déclenche une véritable hantise, générant chez elles à la fois hallucinations, paranoïa, passion et violence. Dans les couloirs de l'établissement, un tueur invisible y sème la terreur.

Set three years before the first film, the action takes place in and around the Meadow View Inn, a small roadside motel in rural North Carolina. It opens with newlyweds driving and the bride wants to have sex in the car, which makes the groom stop. They are then stopped by a man, whose property they are on. The manager, Gordon (David Moscow), oversees an operation of using cameras within the motel to videotape the newlyweds having oral sex. Later, a serial killer (Scott G. Anderson), who checks into the motel as Smith brutally murders a woman he brought with him in one of the motel rooms. The horrified staff see the whole crime on videotape and then subdue the man. They, however, do not know what to do with him as they will endanger their operation if they call the police. Smith manages to convince Gordon and his staff to hire him to torture and murder the motel guests on videotape and then sell them as snuff films.

Detective Anna Manni (Argento) travels to Florence on the trail of a serial killer Alfredo (Thomas Kretschmann). While at a museum, Anna is struck by Stendhal syndrome, which causes people to become overwhelmed by great works of art. Alfredo uses this disorder against Anna, kidnapping and raping her. From their suburban home, Doctor Gordon Mortley and his sister Nurse Edith run Shady Palms Clinic, a facility that takes in physically or mentally ill indigents sent to them by the county. In reality, the Mortleys are con artists who murder their patients (usually during "surgeries" performed on them by Gordon) and continue billing the state for their care afterward. The only permanent resident of the clinic is the alcoholic Louise Kagel, who Gordon supplies with liquor in exchange for sex. One day, a new social services worker named Faith Chandler drops off John Davis, a man afflicted with tuberculosis. Edith smothers John, and Gordon buries his body, though he is later forced to dig it up and crudely puppeteer it to create the illusion that Davis is still alive when Faith asks to check in on him after bringing Charles Bedowski, who has a heart condition, to Shady Palms.
