The story focuses on the three brothers Andrew E., Jimmy Santos, and Keempee de Leon. They live in the slums of Manila as "bakal-dyaryo-bote" collectors recycling scrap metal, newspapers and empty beverage bottles. The movie begins with three kids, Daigo Madoka, Shin Asuka and Gamu Takayama who played together everywhere they went. When they stopped by at a nearby shop, they realized was 6:00pm and headed home. Asuka and Gamu decided to stay at Daigo’s house for a while as they watch the first classic 1966 Ultraman show. Ever since they watched the show, they began to have many dreams and ambitions. Sometime later, when they were playing softball, they witnessed an unusual girl and they also invited her to play. When they launched a paper plane to the sky, it flew towards the star. Asuka wishes to be a professional softball player. Daigo wishes to be a spaceship pilot who will travel to the Ultras’ homeland and Gamu wishes himself as a scientist in order to build Daigo’s spaceship. The girl’s wishes were never revealed as she said farewell to Daigo and walked away, never to be seen again.

Trevor Slattery, having been captured at the end of Iron Man 3, is now being held in Seagate Prison. At the prison he is living luxuriously, with his own personal "butler" Herman, and other inmates acting as his fan club and protection. Slattery is interviewed by documentary filmmaker Jackson Norriss, who wishes to chronicle the events of the Mandarin situation seen in Iron Man 3. Norriss, trying to learn more about Slattery personally, recounts his past from his first casting as a child as well as his starring in a failed CBS pilot. Norriss eventually informs Slattery that his portrayal has angered some people, including the actual Ten Rings terrorist group, which Slattery did not know existed. Norriss tells him the history of the Mandarin and the terrorist group, before revealing that he is actually a member of the group. The real reason for Norriss interviewing Slattery is to break him out of prison so he can meet the actual Mandarin. Hearing this, Slattery still has no idea of the full ramifications of his posing as the Mandarin.

Decades prior to the start of the film, the American military sponsored a superhero group called "Team Zenith." Its leaders were Jack Shepard, a.k.a. "Captain Zoom", who possessed super-speed, and his brother, Connor, a.k.a. "Concussion", who could project sonic blasts. The military tried to increase the team's powers by exposing them to an experimental form of radiation called "Gamma-13". This caused Concussion to become more powerful, but it also reversed his personality. Thinking Zoom and the team betrayed him, Concussion went insane and killed his teammates Marksman, Ace, and Daravia. Concussion was believed to be killed by Zoom in an explosion, but he had been sent into another dimension instead. Zoom lost his powers, and his brother.
