Devant la progression alarmante des toxicomanes parmi la jeunesse, Derek Carpenter, un agent de la CIA, est envoyé au Pérou par la section anti-droguesavec pour mission de détruire la plaque tournante secrète où convergent toutes les récoltes d'opium des plateaux andins. Dans l'avion qui le mène en Amérique Latine, Carpenter sympathise avec Linda Montana, la fille de Don Luis qui n'est autre que sa cible, soit le chef tout puissant d'un réseau de trafiquants de stupéfiants. Ce dernier explique à Linda que son exportation de drogues vers les États-Unis est un acte de vengeance contre les Américains. Selon lui, ancien guérillero communiste, ils sont responsables de la mort de sa femme dans un attentat perpétré contre Che Guevara. Pour le nuire, Derek est prêt à tout mais Linda est partagée entre venger sa mère contre les Américains ou soutenir Derek dans sa mission.

Un chargement de drogue disparaît entre l'Italie et New York. When a fellow agent is murdered, secret agent Miss Maxwell (Lois Maxwell) is sent to find the murdered agent's girlfriend, Miss Yashuko (Yashuko Yama), who is in possession of valuable information. Maxwell discovers that Yashuko is in the care of Dr. Neil Connery, a cosmetic surgeon, hypnotist, and the brother of another important agent; Connery then takes Maxwell into custody as well. After an armored car leaves the bank with ten-million dollars, it is attacked on route by Diabolik (John Phillip Law) who manages to steal the money and escape with his partner Eva Kant (Marisa Mell). Leaving the money in their underground hideout, Diabolik and Eva attend a press conference held by the Minister of the Interior which they disrupt by releasing laughing gas. Due to the high level of crime, the death penalty is brought back. The police cannot find Diabolik but gang leader Valmont (Adolfo Celi) suffers at their hands because of a clampdown due to Diabolik's actions. Realising things can get only worse because of Diabolik's crime spree, Valmont contacts Inspector Ginco (Michel Piccoli) and makes a bargain to catch Diabolik alive for the police. In his hideout, Diabolik decides to steal the famous Aksand emerald necklace for Eva's birthday from the Saint Just Castle. Valmont builds up an identikit picture of Eva and circulates it as a means of capturing Diabolik. Diabolik scales the walls of Saint Just Castle where he finds the police are waiting, Diabolik manages to steal the necklace by fooling the police officers with mirrors on the road and dummy decoys of himself.
