Makoto Katagiri (Yasunori Ishizu) is a 30-year-old man living with his mother Miyo (Hiroko Fuji), the latter who operates a pornographic photography business specializing in sadomasochistic content out of her combined home and adult toy store. As a child, Makoto shot and killed a black American soldier who he found having sex with his mother. The incident left Makoto psychologically stunted as well as impotent, and as such his mother and her staff treat Makoto much as a child. Despite this, Makoto has developed a strong interest in BDSM, though he has no outlet for his interests.

In the early 1990s, a tractor mechanic nicknamed Steelhead (Jackie Chan) illegally enters Japan from China in search of his fiancée, Xiu-Xiu (Xu Jinglei) with the help of his "brother" Jie (Daniel Wu). The film begins with two anti-corruption detectives observing the discovery of dead bodies in a car being recovered from the bottom of a harbor. The detectives suspect that the drowned bodies are related to the recent gang war and power struggle at the huge Sanno-kai crime syndicate, which covers a large portion of eastern Japan. The film begins with a sumptuous banquet at the opulent estate of the Grand Yakuza leader Sekiuchi (Soichiro Kitamura), boss of the Sanno-kai, a huge organized crime syndicate controlling the entire Kanto region, and he has invited the many Yakuza leaders under his control. After the formal conclusion of the banquet, Kato, the chief lieutenant of Sekiuchi, pulls one of the Yakuza leaders, Ikemoto, aside and makes plain that he is displeased with the news that Ikemoto has become friendly with a rival gang leader, Murase, while the two were unexpectedly imprisoned together. Kato orders Ikemoto to bring the unassociated Murase-gumi gang in line, and he immediately passes the task on to his subordinate Otomo (Beat Takeshi), who runs his own crew.
