Charles Kessler (Bela Lugosi) is plagued by homicidal urges. His wife (Betty Compson), who had left him for another man, gets into a car accident that leaves her brain damaged and is kept in the basement, in secret, by Kessler's gardener. When an innocent man is executed for a murder Kessler committed in the house, his twin brother visits and tries to unravel the mystery. He discovers that Kessler is the killer and doesn't know it. His brother subdues him and contacts the police, who arrest Kessler.

The famous Dr. Ernest Sovac's best friend, bookish college professor George Kingsley, is run down while crossing a street. In order to save his friend's life, Sovac implants part of another man's brain into the professor's. Unfortunately, the other man was a gangster who was involved in the accident. The professor recovers but at times behaves like the gangster, and his whole personality changes. Sovac is horrified but also intrigued, because the gangster has hidden $500,000 somewhere in the city. The doctor continues to treat his friend and, when the professor is under the influence of the gangster's brain, Karloff attempts to have the man lead him to the fortune. Béla Lugosi plays a gangster also trying to get his hands on the cash.

In London, Dr. Orloff (Bela Lugosi) runs an insurance agency where he loans money in exchange for the assigning of life insurance benefits to himself of many of his customers' policies. A resident of Orloff's (under the name "Dearborn") home for the "Destitute Blind" is Jake, a tall, hideous brute, who murders on Orloff's behalf. Scotland Yard begins finding bodies in the Thames River. One of the dead men has a daughter named Diane (Greta Gynt) for whom Orloff obtains employment at the home for the blind. Suspicions begin to arise surrounding Dearborn and Orloff in relation to the dead bodies. Orloff sends Jake (Wilfred Walter) to kill Diane who has found out too much about them. When Orloff disappears, Diane finds one of her father's cuff links in the home from the blind. Confronted by Diane, Dearborn removes his disguise to show himself as Orloff. Orloff traps Diane with a straight-jacket and calls for Jake to finish the job. Jake refuses as he has found out that Orloff has murdered a blind friend of his. Jake turns on Orloff and throws him out of a window allowing him to sink in the mud below.
