House of Evil is one of four low-budget Mexican horror films Karloff made in a package deal with Mexican producer Luis Enrique Vergara. The others are The Snake People, The Incredible Invasion, and Fear Chamber. Karloff's scenes for all four films were directed by Jack Hill in Los Angeles in the spring of 1968. The films were then completed in Mexico.

Stephen Arrowsmith (Paul Muller), a scientist experimenting with the possibility of regenerating blood through electricity has his home and laboratory in the castle owned by his wife Muriel (Barbara Steele). Arrowsmith finds her later having sex with a gardener and disfigures him with a hot poker and burns Muriel's face with acid before electrocuting both of them. Howard Thorne is a rapist in Los Angeles: he meets women at work and at parties or he sees them walking down the street, and he follows them, terrifies them, and assaults them. He also dreams about these assaults, and he's unclear how much of what he's done is real and how much is fantasy. He ignores his heroin-using wife, Vicki, who tries everything she can think of to get his sexual attention. In Venice, California, student Daisy (Merissa Mathes) leaves a club alone after having an argument with her beatnik boyfriend Max (Carl Schanzer). Walking through the deserted streets, she stops to admire some gruesome paintings in a gallery window painted by artist Antonio Sordi (Campbell), who coincidentally also comes by to look in on his "lost children." After a friendly conversation, Sordi convinces the young woman to pose nude for him that night. At his bell-tower studio, Sordi is possessed by the spirit of a long-dead ancestor and suddenly transforms into a vampiric monster who hacks the screaming Daisy to death with a cleaver. Afterwards, he lowers her mutilated corpse into a vat of boiling wax.

Blossom (Pam Grier), a buxom bad girl, is the rough-and-ready girlfriend of a radical guerrilla leader, Django (Sid Haig). She is now forced to deal with crazy inmates, gay guards, and torture of the cage. Terry, Blossom, and Django (who busted in by seducing Rocco, one of the guards) eventually come together to face off against the warden Zappa (Andrés Centenera) to stage an explosive breakout.
