Nomadic, flaky Caroline Wolff (Ellen Barkin) wants to settle down in one place, find a decent man, and provide a better home for her and her son, Tobias "Toby" Wolff (Leonardo DiCaprio). When she moves to Seattle, Washington, and meets the seemingly respectable Dwight Hansen (Robert De Niro) (in real life Dwight Thompson worked as a mechanic for Seattle City Light at the time), she thinks she has accomplished her goals. Toby, however, comes to feel differently; Dwight's true personality is revealed after Toby spends a few months separated from his mother with Dwight and his children. The boy's stepfather-to-be seems to want to mold Toby into a better person, but his method includes emotionally, verbally and physically abusing the boy.

The title designates time and location: an unusually hot August in a rural area outside Pawhuska, Oklahoma. Beverly Weston (Sam Shepard), an alcoholic, once-noted poet, interviews and hires a young native American woman Johnna (Misty Upham) as a live-in cook and caregiver for his strong-willed and contentious wife Violet (Meryl Streep), who is suffering from oral cancer and addiction to narcotics. Shortly after this, he disappears from the house, and Violet calls her sister and daughters for support. Her sister Mattie Fae (Margo Martindale) arrives with husband Charles Aiken (Chris Cooper). Violet's middle daughter Ivy (Julianne Nicholson) is single and the only one living locally; Barbara (Julia Roberts), her oldest, who has inherited her mother's mean streak, arrives from Colorado with her husband Bill (Ewan McGregor) and 14-year-old daughter Jean (Abigail Breslin). Barbara and Bill are separated, but they put up a united front for Violet.

Set in 1964 at a Catholic church in The Bronx, New York, the film opens with the jovial Father Flynn (Philip Seymour Hoffman) giving a sermon on the nature of doubt, noting that like faith, doubt can be a unifying force. The next evening, Sister Aloysius Beauvier (Meryl Streep), the strict principal of the parish school, discusses the sermon with her fellow nuns, the Sisters of Charity of New York. She asks if anyone has observed unusual behavior that would inspire Father Flynn to preach about doubt, and instructs them to keep their eyes open should any such behavior occur in the future.

The film centers around the three children of Arthur (E. G. Marshall), a corporate attorney, and Eve (Geraldine Page), an interior decorator. Renata (Diane Keaton) is a poet whose husband Frederick, a struggling writer, feels eclipsed by her success. Flyn (Kristin Griffith) is a vain actress who is away most of the time filming; the low quality of her films is an object of ridicule behind her back. Joey (Mary Beth Hurt), who is in a relationship with Mike (Sam Waterston), cannot settle on a career, and resents her mother for favoring Renata, while Renata resents their father's concern over Joey's lack of direction.

Actress Suzanne Vale (Meryl Streep) is a recovering drug addict trying to pick up the pieces of her acting career and get on with her life after being discharged from a rehab center to kick a cocaine-acid-Percodan habit; after overdosing while on a date, her mother admitted her to the rehab center from the emergency room. When she is ready to return to work her agent advises her the studio's insurance policy will cover her only if she lives with a "responsible" individual such as her mother Doris Mann (Shirley MacLaine) who was the reigning musical comedy star of the 1950s and '60s. Suzanne is very reluctant to return to the woman she struggled to escape from for years after growing up in her shadow. The situation is not helped by the fact that Doris is loud, competitive, manipulative, self-absorbed and given to offering her daughter unsolicited advice with insinuating value judgments while treating her like a child.

In the small town of Endora, Iowa, Gilbert Grape (Johnny Depp) is busy caring for Arnie (Leonardo DiCaprio), his brother with a developmental disability, as they wait for the many tourists' trailers to pass through town during their "yearly ritual" of camping at a nearby recreational area. His mother, Bonnie (Darlene Cates), gave up on life after her husband hanged himself seventeen years earlier. She spends almost all of her time on the couch watching TV. With Bonnie unable to care for her children on her own due to her morbid obesity, Gilbert has taken responsibility for repairing the old house and looking after Arnie, who has a habit of climbing the town water tower, while his sisters Amy and Ellen do the rest. The relationship between the brothers is of both care and protection, as Gilbert continually enforces the "nobody touches Arnie" policy. A new FoodLand supermarket has opened, threatening the small Lamson's Grocery where Gilbert works. In addition, Gilbert is having an affair with a married woman, Betty Carver (Mary Steenburgen).

In the fictional "Verona Beach", the Capulets and the Montagues are arch-rivals. The animosity of the older generation—Fulgencio and Gloria Capulet and Ted and Caroline Montague—is felt by their younger relatives. A gunfight between the Montague boys led by Benvolio, Romeo's cousin, and the Capulet boys led by Tybalt, Juliet's cousin, creates chaos in the city. The Chief of Police, Captain Prince, reprimands the families, warning them that if such behavior continues, their lives "shall pay the forfeit of the peace".
