Rose Elliot (Miracle), a poet living alone in New York City, finds an ancient book called The Three Mothers. It tells of three evil sisters who rule the world with sorrow, tears, and darkness. The head of a now abandoned asylum, Dr. Richard B. Vannacutt (Jeffrey Combs), once performed grotesque experiments on the patients, killing many in the process. The hospital was closed in 1931 after the patients escaped from their cells, killing the entire staff (except five who were not present) and setting fire to the hospital.
