A couple, Jared (Judd Lormand) and Lily (Boyana Balta), finds a demonic-looking Nell Sweetzer (Ashley Bell) squatting next to their refrigerator. She soon is taken to a hospital, where she appears to be catatonic. After spending a few months at Frank's (Muse Watson) home for girls in New Orleans and settling in as a chambermaid at a hotel under the supervision of her boss Beverly (Diva Tyler), Nell's condition seems to have improved and she no longer has "bad dreams". Nell and her group of friends Gwen (Julia Garner), Daphne (Erica Michelle), and Monique (Sharice Angelle Williams) attend a Mardi Gras parade; Nell witnesses many strange happenings there, including masked men watching her. Minneapolis car salesman Jerry Lundegaard (William H. Macy) is desperate for money. Shep Proudfoot (Steve Reevis), an ex-convict, gives him a name; he travels to Fargo, North Dakota, where he hires two men (Carl Showalter (Steve Buscemi) and Gaear Grimsrud (Peter Stormare)) to kidnap his wife, Jean (Kristin Rudrüd), and ransom her for $80,000, knowing his wealthy father-in-law and boss, Wade Gustafson (Harve Presnell) will pay. Kolyma and Gagarin are two boys in a small village of Transnistria, who are raised by Kolyma's grandfather Kuzja. Kuzja imposes very strict education to the children, focusing on hatred for the Soviets, regarded as enemies. In a robbery by Kolyma and Gagarin, and their friends Mel e Vitalic other boys against Soviets, Gagarin is stopped and arrested. Seven years later he is freed, but he discovers that his world has completely changed, and he does not know how to succeed in solving his problems. Gagarin finally discovers that the ideals of his people have collapsed in the drug trade, and so he comes into this new system, but he ends up in strong contrast with Kolyma.

Duchovny plays Eugene Sands, a surgeon who has his medical license revoked after operating under the influence of amphetamines and opiates. Hutton's character, a crime lord named Raymond Blossom, happens upon him in a bar where Sands saves someone's life with an emergency procedure to inflate a collapsed lung. Blossom hires Sands as his personal physician, patching up his accomplices when they cannot go to a hospital, and tending to the crime boss and his girlfriend, Claire (Jolie). In the final act of the film, Claire and Sands become involved, and he must face up to conflicting loyalties to Blossom, Claire, and the FBI agent who has blackmailed him into being an informant.
