In 1923 Tennessee, two young boys, Rafe McCawley (Jesse James) and Danny Walker (Reiley McClendon), play together in the back of an old biplane, pretending to be soldiers fighting the Germans in World War I. After Rafe's father lands his biplane and leaves, Rafe and Danny climb into the plane and Rafe accidentally starts it, giving the boys their first experience at flight. Soon afterward, Danny's father (William Fichtner) comes to take him home. He calls Rafe a "stupid boy" and beats Danny. In an effort to protect Danny, Rafe hits Danny's father with an old propeller and calls him a "dirty German". Danny's father reacts by saying he fought the Germans in the trenches in France during World War I, and that he prays that no one will ever have to see the things he saw.

Lieutenant Jake "Cool Hand" Grafton (Brad Johnson) and his bombardier/navigator and best friend Lieutenant Morgan "Morg" McPhearson (Christopher Rich) are flying a Grumman A-6 Intruder over the Gulf of Tonkin towards North Vietnam. They hit their target, a suspected truck park which actually turns out to be trees. Morg is fatally shot in the neck by a Vietnamese peasant. Landing on the USS Independence with Morg dead, a disturbed Jake, covered in blood, walks into a debriefing with Commander Frank Camparelli (Danny Glover) and Executive Officer, Commander "Cowboy" Parker (J. Kenneth Campbell). Camparelli tells Jake to put Morgan's death behind him and to write a letter to Sharon, Morg's wife. New pilot Jack Barlow (Jared Chandler), nicknamed "Razor" because of his youthful appearance, is then introduced.

In 1991, Thomas Hardy (Bruce Willis), a Pittsburgh Police homicide detective, has just broken the Blue Code of Silence by informing on his partner and cousin, Jimmy Detillo (Robert Pastorelli), for using excessive force on a suspect. He attends the Policemen's Ball with his father, Vince Hardy (John Mahoney). However, the ball is postponed after a police scan goes out indicating a serial killer nicknamed the Polish Hill Strangler has been spotted on the 31st Street Bridge. While pursuing the killer, Tom gets into an accident and is knocked unconscious. When he regains consciousness, he learns his father has been shot dead and the killer has escaped. Police make an arrest based on the testimony of Chick Chicanis (Gareth Williams). Meanwhile, rather than go to prison, Jimmy climbs to the top of the 31st Street Bridge and jumps off. His body is never found.

The film opens with a review of U.S. Customs Agent, Victor "Vic" Cooper's (Baldwin) personnel file, which shows that he struggles to disassociate his undercover identities from his real one. In 1993, following the ousting of the central government and start of a civil war, a major United Nations military operation in Somalia is authorized with a peacekeeping mandate. After the bulk of the peacekeepers are withdrawn, the Mogadishu-based militia loyal to Mohamed Farrah Aidid have declared war on the remaining UN personnel. The film's plot follows a traditional theme, with Zatoichi (a blind swordsman) coming to the defense of townspeople caught up in a local Yakuza gang war and being forced to pay excessive amounts of protection money. Meanwhile, Zatoichi befriends a local farmer and her gambler nephew and eventually offers his assistance to two geisha siblings (one of whom is actually a man) who are seeking revenge for the murder of their parents. The siblings are the only survivors of a robbery and massacre that was carried out on their family estate ten years ago. They soon discover the people responsible for the murders are the same Yakuza wreaking havoc on the small town.
