Five friends are traveling through rural Nebraska on a weekend getaway when their SUV is suddenly attacked and forced off the road by a flock of wild crows. The Archangel Michael (Paul Bettany) falls to Earth in Los Angeles and cuts off his wings. He steals a police car after a policeman is killed by another officer who is possessed. He travels towards the Paradise Falls Diner, near the edge of the Mojave Desert. Meanwhile, Kyle (Tyrese Gibson), a single father driving to Los Angeles, stops at the diner. He meets the owner, Bob Hanson (Dennis Quaid); Bob's son, Jeep (Lucas Black); Percy (Charles S. Dutton), the short-order cook; Charlie (Adrianne Palicki), a pregnant waitress; Howard (Jon Tenney) and Sandra (Kate Walsh) Anderson, a married couple; and Audrey (Willa Holland), their rebellious teenage daughter. An old woman (Jeanette Miller) enters the diner and begins taunting the diner's patrons. When Howard confronts her, she rips his throat open, screeches that they are all going to die, and climbs up the ceiling. Kyle shoots her before she can kill Jeep. Percy, Kyle, Sandra, Audrey and Charlie try to get Howard to the hospital but they are forced to turn back after passing through a swarm of flies.

Psychiatrist Dr. Miranda Grey (Halle Berry) works at a mental hospital and has a car accident after trying to avoid a girl (Kathleen Mackey) on a road during a stormy night while driving back home. She rushes to try to help the girl. The girl turns out to be a ghost and possesses Miranda's body by burning her after she extends her hand to the girl. Miranda loses consciousness. Miranda next wakes up in the very hospital she works for, but as a patient treated by her co-worker, Dr. Pete Graham (Robert Downey, Jr.). Drugged and confused, she remembers nothing of what happened after the car accident. To her horror, she learns that her husband Douglas (Charles S. Dutton) was brutally murdered and that she is the primary suspect. While Miranda copes with her new life in the hospital, the ghost uses her body to carry out messages (most noticeably, she carves the words "not alone" into Miranda's arm), which leads her former colleagues to believe Miranda is suicidal and is inflicting the wounds on herself.

In New York City, avant garde sculptor Lily McGuire (Lesley Ann Warren) lives a complicated and frustrating life as she tries to not only provide for herself and her daughter, but debut her latest artwork known as Apology. The design is a two part project consisting of a walkthrough sculpture of advanced mechanical design and a phone service that allows callers to anonymously leave confessions of whatever they desire on the answering machine. There is just one problem: someone has been calling the line and using it to announce a string of recent high-profile killings. After contacting the police, Detective Hungate (Peter Weller) advises Lily to take the threats seriously. Eventually, the serial killer stops his phone calls and intends to murder Lily to the sounds of the exhibits programmed confessions. Charles S. Dutton, Harvey Fierstein, and Chris Noth co-star.
