The film follows Tommy Basilio (Buscemi), an alcoholic and fixture at the local bar, Trees Lounge, who begins to float after losing his girlfriend of eight years Theresa (Elizabeth Bracco) and his job as a mechanic. His Uncle Al (Seymour Cassel) dies at the wheel of his ice-cream truck while out on his rounds and at the wake following his funeral he indulges in lines of cocaine with his brother and cousins. Tommy takes them to the Trees Lounge to carry on drinking, where a drunken brawl breaks out between his cousin and one of the regulars, Mike (Mark Boone Junior). In the next scene Mike and Tommy are driving away from a late night convenience store after buying more beer. They are discussing how Tommy stole money from Rob (Anthony LaPaglia), the owner of the garage where he lost his job. They discuss how Rob is seeing Tommy's ex-girlfriend who is pregnant and Tommy might be the father but she is telling Rob otherwise.

In New York City, soft-spoken Helen returns to the apartment she shares with her boyfriend Marco after enduring an unhygienic and inept abortion. Soon after, when Helen is hospitalized for profuse bleeding, Bobby sneaks in after visiting hours and tells her that Marco has gone away. Briefly, Helen considers returning to her dysfunctional family in Fort Wayne, Indiana. However, after she is released from the hospital, she moves in with Bobby. As they are walking together, Bobby steals a television out of a parked van and pawns it for money to buy drugs. At his apartment, Bobby declines Helen's invitation to have sex until she feels better. When she awakens to find him taking drugs, he explains that he is not an addict, but only "chipping.

In mid-2000s, the illegal drug trade based in Kuala Lumpur is booming. The Malaysian authorities are called upon for assistance by their Indian counterparts. The Indian team sent to Kuala Lumpur is headed by Deputy Commissioner of Police D'Silva (Boman Irani), who targets the illegal drug operations of Singhania (Rajesh Khattar). Helping him along the way is Inspector Vishal Malik (Om Puri), an Indian officer from Interpol. Both of them know that in order to put an end to the illegal drug trade, they must capture "Don" (Shah Rukh Khan) who manages Singhania's business in Kuala Lumpur. Singhania was one of the two closest and most trusted men of Boris, a criminal who went underground and was presumed dead by the police. Boris's other lieutenant was a man called Vardhan, equally ruthless as Singhania, but his whereabouts are unknown. The police thinks that Singhania had killed Vardhan upon learning that Boris was about to hand over his drug business to Vardhan.
