On a beach in Australia, two American surfers say goodbye to each other as one is about to leave for his flight back to California. The two men don't notice the large number of dead birds on the beach. On the flight, the surfer returning home starts coughing up blood and dies before the plane can land. Back in Australia, the other surfer is found dead in his apartment. Kayla Martin (Tiffani Thiessen) is a medical doctor with the Centers for Disease Control and Prevention (CDC) who has been assigned to the case. She and the local officials carry the body of the surfer out of the plane and start processing people to be put in quarantine until they can be evaluated for exposure. During the transfer, one of the passengers escapes to complete a business deal and unknowingly spreads the virus throughout Los Angeles. It is also revealed that outbreaks of the so-called "riptide virus" (as it comes to be known), an offshoot of the bird flu, are occurring in other cities around the world.

In 1958, two U.S. Air Force pilots and aspiring astronauts, William "Hawk" Hawkins (Tommy Lee Jones) and Frank Corvin (Clint Eastwood), are testing a modified X-plane when Hawk decides to try to break a height record. The plane stalls and they are forced to eject, narrowly missing a B-50 Superfortress piloted by navigator "Tank" Sullivan (James Garner) as they parachute to safety. On the ground, Frank punches Hawk for putting their lives at risk, but their fight is broken up by engineer Jerry O'Neill (Donald Sutherland). Their supervising officer, Bob Gerson (James Cromwell), chastises Hawk for his recklessness, before taking them to a press conference, where it is announced that the Air Force will no longer be involved in space flight tests as this has now been handed off to the newly created NASA, ending the four's dreams of reaching space.

A group of vampires look for the hidden resting place of Drake, also known as Dracula—the genetic "father" of their species. After being impaled by a pipe and plunging into a river at the end of the previous film, the undead Maniac Cop Officer Matthew Cordell acquires a junked police cruiser and continues his killing spree through New York City. Finding a convenience store in the middle of a robbery, he kills the clerk; the thief is subsequently killed in a shootout with police. As Cordell stalks the streets, his enemies Officers Jack Forrest and Theresa Mallory are put back on duty by Deputy Commissioner Edward Doyle, who has the two undergo a psychiatric evaluation under Officer Susan Riley. While Jack is content that Cordell is long gone and wants to go on with his life, Theresa is convinced that Cordell is still alive and plotting his revenge.
