Young and handsome Sergio (Ricardo Meneses) works the night shift as a trash collector in Lisbon. He is uninterested in his pretty female co-worker Fatima, who displays an avid interest in him, so instead Sergio roams the city. Eventually Sergio becomes fascinated with a sleek motorcycle and its arrogant owner - a young man totally indifferent to Sergio. Sergio's surfacing desires unleash his darkest impulses, sending him down a dangerous path of violence, depravity, and degradation.
The film chronicles Flanagan for several years leading up to his death in 1996. It explores various aspects of his life, artwork, and philosophy through interviews and other personal footage depicting Flanagan, his partner Sheree Rose, and the Flanagan family. Sick also features Flanagan's home movies, performance videos, and video diaries, as well as an excerpt of Flanagan's performance in the music video for "Happiness in Slavery" by Nine Inch Nails.
A married couple and their three young adult children, a son and two daughters, live in a large compound with a garden and swimming pool. A tall fence surrounds the property and the children have never been on the other side of it, for their parents have kept them unaware of the outside world, even of the existence of the telephone. They are taught different meanings for everyday words. The parents promise that each child will be ready to venture outside the compound once she or he has lost a dogtooth. Although the children are told they have a brother just on the other side of the barrier, he never appears.

The lead characters of the Story of O and Retour à Roissy novels, Sir Stephen and O, are placed in southern China where Sir Stephen owns a casino. Sir Stephen places O in a Chinese brothel for "training" and O is then subjected to a variety of humiliating experiences to prove her unconditional obedience. A sub-plot concerns a coolie rebellion due to the resentment towards Europeans by the local population and a young man desperate to afford O's favors at the brothel.
An arrogant wealthy capitalist named Raffaella (Mariangela Melato) is vacationing on a yacht in the Mediterranean Sea with friends—swimming, sunbathing, and talking incessantly about the virtues of her class and the worthlessness of the political left. Her nonstop political monologue infuriates one of the underclass deckhands on her yacht, Gennarino (Giancarlo Giannini), a dedicated Communist who manages to restrain his opinions to avoid upsetting his boss and losing his good job. Despite her humiliating insults, Gennarino agrees to take her out on a dinghy late in the evening to see the rest of her friends who have gone ahead without her. On their way, the outboard motor gives out, leaving them stranded in the middle of the sea with no land in sight.
A young woman fashion photographer known only as O is taken by her lover René to Château Roissy, where she is subject to various sexual and/or sadomasochistic rituals. O is taught to be constantly available for oral, vaginal, and anal intercourse. She is regularly stripped, blindfolded, chained and whipped;her labium is pierced and her buttocks are branded. She leaves wearing a ring as a sign of her initiation.
In 1936 Tokyo, Sada Abe (Eiko Matsuda) is a former prostitute who now works as a maid in a hotel. The hotel's owner, Kichizo Ishida (Tatsuya Fuji), molests her, and the two begin an intense affair that consists of sexual experiments and various self-indulgences. Ishida leaves his wife to pursue his affair with Sada. Sada becomes increasingly possessive and jealous of Ishida, and Ishida more eager to please her. Their mutual obsession escalates to the point where Ishida finds she is most excited by strangling him during lovemaking, and he is killed in this fashion. Sada then severs his penis, walks around with it inside her for several days, and writes, "Sada Kichi the two of us forever," in blood on his chest.
Boy (Takuya) meets girl (Satsuki), they fall in love. When she discovers his fetishes Satsuki brands Takuya hentai (pervert) and leaves him, only to be drawn back by the power his fetishes give her over him.
A teenage girl who was molested by her father calls David Kelley, a radio psychologist working for KDRX, and shoots herself on the air. Later, a real estate broker shows off a house to prospective buyers, and discovers the decomposing remains of another realtor in the washroom, the fourth victim of a psychopath dubbed the "Open House Killer". Outside Grant Real Estate, which David's girlfriend Lisa runs, someone digs through the trash, and takes discarded Seller Listings. The vagabond goes to one of the listed houses, and murders the realtor and buyer inside with a plunger that has had razor blades attached to it. The Open House Killer (who gives his name as "Harry") then calls David at KDRX, and opines that his victims deserved their fates.
