At 30 years old, Opie (James Ricardo) is a virgin whose existence is made up of watching pornography and eating junk food. One day a drug dealer named Thai (April Wade) mistakenly comes to his door. She decides to help Opie by finding him a good woman, initially (and unsuccessfully) by online personal ads. Thai and Opie end up sleeping together while high on marijuana, and then end up seeing each other regularly. Opie starts having sex with other women, including Thai's lesbian lover Dakota (Ute Werner) and a "gun-toting" nymphomaniac named Rain (Jesselynn Desmond).

Unlike the first two Creepshow installments, in which the wraparound element linking the stories was a horror comic, Creepshow III takes an approach similar to Quentin Tarantino's Pulp Fiction, in which characters from each story collide with each other during the film. In 1968, an infected man from Russia, Rubinsky, is being operated on at a military hospital when soldiers arrive and begin to shoot all of those who are in the building. One of the hospital students, DeLuca, steals a strange vial, enclosing it in his thermos. He is later shot by the soldiers as he attempts to escape through the woods and the thermos is dropped. The soldiers in the hospital continue to kill any one that has been in contact with the infected man. Zombies kill three of the soldiers and multiple hospital orderlies. The rest of the zombies are killed when C4 is detonated by sharpshooters outside the hospital.
