Dr. John H. Watson (Robert Duvall) becomes convinced that his friend Sherlock Holmes (Nicol Williamson) is delusional—particularly in his belief that Professor James Moriarty (Laurence Olivier) is a criminal mastermind—as a result of his addiction to cocaine. Indeed, Moriarty visits Watson to complain about being harassed by Holmes. Watson enlists the aid of Sherlock's brother, Mycroft (Charles Gray), to trick Holmes into traveling to Vienna, where he will be treated by Sigmund Freud (Alan Arkin). Framing Story Five strangers board a descending lift, one by one, in a modern office block in London. They reach the sub-basement, though none of them have pressed for that destination. There they find a large, elaborately furnished room which appears to be a gentlemen's club. The lift door has closed and there are no buttons to bring it back, nor any other exit. Resigned to waiting for help, they settle down with drinks and talk. The conversation turns to dreams, and each man tells of a recurring nightmare.

The film is set in 1918, in which a war widow Mrs. Allen (Dorothy Alison) and her three children, Lucy (Lynne Frederick), Jamie (Garry Miller) and baby Benjamin are reduced to living in a tiny, squalid, Camden Town flat. Just before Christmas, a mysterious old man, Mr. Frederick Percival Blunden (Laurence Naismith) visits the family, introducing himself as a representative of a firm of solicitors. The family are told there is an opportunity to become the caretakers of a derelict country mansion in the Home Counties named Langley Park, which was gutted by fire years before, and is now in the charge of the solicitors. Mrs. Allen's instructions are to care for the property until such time as the heirs to the estate can be traced. In desperation, Mrs. Allen agrees to take up the post. There are rumours that the house is haunted, and as a result few locals venture up to the property.
