After the McManus brothers, Connor and Murphy (Sean Patrick Flanery and Norman Reedus), and their father, Noah (a.k.a. "Il Duce") (Billy Connolly), assassinated Joe Yakavetta, they fled to Ireland. Eight years later, their uncle Sibeal (Mairtin O'Carrigan), a priest, arrives to inform them that a renowned Boston priest was murdered by a mysterious assassin who attempted to frame the Saints by using their ritual assassination style. Years after a zombie apocalypse, survivors have set up outposts across the United States, one of which, in the city of Pittsburgh, Pennsylvania, contains a feudal-like government. Bordered on two sides by rivers and on the third by an electric fence, the city has become a sanctuary, with the rich and powerful living in a luxury high-rise called Fiddler's Green while the rest of the population subsists in squalor. The city's ruler Paul Kaufman (Dennis Hopper) has sponsored Dead Reckoning, a heavily armored vehicle that can travel through the zombie-infested areas with ease. Armed with remote-controlled heavy machine guns and video cameras, Dead Reckoning functions as a moving fireworks launch base, on the premise that zombies are fascinated by fireworks and will stare at them, ignoring their environment. Riley Denbo (Simon Baker), designer and commander of Dead Reckoning, has recently retired. Unlike Kaufman, Riley is respected for his work in protecting the city from danger, as well as for bringing critical food and medical supplies which the citizens can no longer acquire safely themselves.

After finishing a long shift, nurse Ana returns to her suburban neighborhood and her husband, Luis. Caught up in a scheduled date night, they miss an emergency news bulletin. The next morning, a neighborhood girl enters their bedroom and kills Luis, who immediately reanimates as a zombie and attacks Ana. She flees in her car but crashes and passes out. Upon waking, Ana joins with Police Sergeant Kenneth Hall, electronics salesman Michael, petty criminal Andre and his pregnant wife, Luda. They break into a nearby mall and are attacked by a zombified security guard, who scratches Luda. They are also confronted by three living guards — C.J., Bart, and Terry — who make them surrender their weapons in exchange for refuge. They split into groups to secure the mall and then go to the roof, where they see another survivor, Andy, who is stranded alone in his gun store across the zombie-infested parking lot.

Hansel Schmidt is an East German "slip of a girly boy" who loves rock music, and is stuck in East Berlin until he meets Luther Robinson, an American soldier. Luther falls in love with Hansel and the two decide to marry. This plan will allow Hansel to leave communist East Germany for the capitalist West. However, in order to be married, the couple must consist of a man and a woman. Hansel's mother, Hedwig, gives her child her name and passport and finds a doctor to perform a sex change. The operation is botched, however, leaving Hansel – now Hedwig – with a dysfunctional one-inch mound of flesh between her legs, the eponymous "Angry Inch".

At Culver University in Virginia, General Thunderbolt Ross meets with Dr. Bruce Banner, the colleague and boyfriend of his daughter Betty, regarding an experiment that Ross claims is meant to make humans immune to gamma radiation. The experiment — part of a World War II era "super soldier" program that Ross hopes to recreate — fails, and the exposure to gamma radiation causes Banner to transform into the Hulk for brief periods of time, whenever his heart rate rises above 200. The Hulk destroys the lab and injures or kills the people inside. Banner becomes a fugitive from the U.S. military and Ross in particular, who wants to weaponize the Hulk process.

U.S. Navy SEAL Lieutenant Shane Wolfe is assigned to rescue Howard Plummer, a man working on a top-secret government project from a group of Serbian rebels. Wolfe and his team manage to get Plummer off an enemy boat; moments later, Wolfe and Plummer are shot while boarding the escape helicopter. Plummer is killed in the attack and Wolfe spends two months in the hospital. Wolfe's commanding officer, Captain Bill Fawcett, is assigned to escort Plummer's widow, Julie, to Zürich, where a safety deposit box belonging to the Plummers has been discovered. Orin Boyd (Steven Seagal) is a cop in Detroit's 21st precinct, who saves the Vice President of the United States (Christopher Lawford) from a right-wing Michigan militant group trying to kill him. As Boyd saved the Vice President's life by disobeying orders and killing all the militants, Captain Frank Daniels (Bruce McGill), who is also a long-time friend of Boyd, is forced to fire Boyd, but still tells him to talk with police chief Hinges (Bill Duke) who dislikes Boyd (because Boyd gives the police a bad name by disobeying orders) and has repeatedly tried to fire him, only for Boyd to be returned to duty by his friends in the trade union. Despite Boyd once again being returned to duty, Hinges transfers Boyd to the 15th precinct — Detroit's arguably worst police precinct.

In 1944 German-occupied Poland, 13-year-old Erik Lehnsherr is separated from his parents upon entering a concentration camp.
