In 1917, Baby Jane Hudson is a vaudevillian child star while her sister Blanche Hudson is not famous and overlooked by their father. By 1935, both sisters are movie actors, but Blanche has achieved stardom, while Jane’s films have flopped, leading Jane to drink heavily. One night, returning from a party, one of the sisters (implied to be Blanche) gets out of the car to open the garage door, while the driver (implied to be Jane) attempts to run her over, misses, and crashes into the garage. The story is framed by Mildred's interrogation by police after they discover the body of her second husband, Monte Beragon. The film, in noir fashion, opens with Beragon (Zachary Scott) having been shot. He murmurs the name "Mildred" before he dies. The police tell Mildred (Joan Crawford) that they believe the murderer is her first husband, Bert Pierce (Bruce Bennett). Bert has already been interrogated, and confessed to the crime. Mildred protests that he is too kind and gentle to commit murder, and goes on to relate her life story in flashback.

Doctor Otternschlag (Lewis Stone), a disfigured veteran of World War I and a permanent resident of the Grand Hotel in Berlin, wryly observes, "People come and go. Nothing ever happens", after which a great deal transpires. Baron Felix von Geigern (John Barrymore), who squandered his fortune and supports himself as a card player and occasional jewel thief, befriends Otto Kringelein (Lionel Barrymore), a meek accountant who, having discovered he is dying, has decided to spend his remaining days in the lap of luxury. Kringelein's former employer, industrialist General Director Preysing (Wallace Beery), is at the hotel to close an important deal, and he hires stenographer Flaemmchen (Joan Crawford) to assist him. She aspires to be an actress and shows Preysing some magazine photos for which she posed, implying she is willing to offer him more than typing if he is willing to help advance her career.

When Lord Francis Kelton (Frank Morgan) finds a beautiful woman in his stateroom, he is flustered, but his playboy friend, Lord Arthur Dilling (Robert Montgomery), is fascinated by her.
