The Man with No Name—"Monco"/"Manco"— and Colonel Douglas Mortimer—the "Man in Black"—are two bounty hunters in pursuit of "El Indio," one of the most wanted fugitives in the Wild West, and his gang. El Indio is ruthless, clever, and brutal. He has a musical pocketwatch that he plays before engaging in gun duels: "When the chimes finish, begin," he says. Flashbacks reveal that El Indio took the watch from a young woman whom El Indio found with her lover (in Joe Millard's novelization of the film, her newly wed husband), killed him, and raped her; she killed herself while being raped. The movie revolves around the exploits of Captain Nolan (Harris), an Irish Canadian who catches marine animals to pay off the mortgage of his boat, and return to Ireland. Nolan's crew is looking for a great white shark for a local aquarium, but a scientist named Ken (Carradine) is being targeted by the shark. An orca comes and kills the shark, saving Ken. This switches Nolan's target to the orca. While Nolan is on the journey with his crew, he tries to capture what he believes to be a bull orca, but mistakenly harpoons a pregnant female. Nolan and his crew get the orca on board, where she subsequently miscarries. The crew hoses the dead fetus overboard as the male looks on screaming.

The film begins in Sicily in 1940 during World War II just as Italy enters the war. A young boy, 12-year-old Renato, experiences three major events in one day: First, Italy goes to war; second, he gets a new bike; and third, he first sees the beautiful lady, Malena. Malena's husband, Nino Scordia, has been taken away to fight in Africa and Malena is left alone with her father, an elderly and almost-deaf man. Malena tries to cope with her loneliness, as the town she has moved to tries to deal with this beautiful woman who gets the attention of all the local men, including Renato. However, in spite of the gossip, she continues to be faithful to her husband. Renato becomes obsessed with Malena and starts fantasizing about her. His fantasies become increasingly elaborate and he becomes obsessed with the shy young woman, peeping in her window often as she waits sadly for her beloved husband to return. Renato eventually steals Malena's underwear and begins to fantasize about her in bed, to the horror of his parents. They do everything to stop his behavior, but it is all in vain.
