The film depicts the relatively primitive medical practices of the time and the suppositions that physicians made in their efforts to understand the human body. After King George III begins to go mad, his doctors attempt cures such as blistering and purges, led on particularly by the Prince of Wales' personal physician, Dr. Warren. Meanwhile, another of the King's physicians, Dr. Pepys, analyses the King's stool and urine believing that body wastes may contain some clue to the Royal malady; of course, none of these attempts to cure the King actually works. Finally, Lady Pembroke, attendant to the Queen, recommends Dr. Willis, an ex-minister who attempts to cure the insane through behaviour modification, and who begins his restoration of the King's mental state by enforcing a strict regime of strapping the King into a waistcoat and restraining him whenever he misbehaves.

As the novel unfolds, Allington is beset by a number of difficulties, including his father's death by stroke at dinner one night, and a drinking problem that causes hypnagogic jactitation and hallucinations; Maurice compounds his problems by pursuing an affair with his doctor's wife, neglecting his daughter Amy (whose mother, Maurice's first wife, was run down in a road accident) and attempting to seduce both his current wife and his mistress into a ménage à trois, which backfires when the two women take an enthusiastic interest in each other and effectively shut him out of the orgy.

Teenagers Sarah (Weaver) and Debbie (Murdock) run away from home, before meeting shortly after arriving in London. Debbie, the older and more streetwise of the two, is running from a generally bad home life, where her family is poor and her father drinks excessively. Sarah is running from a more affluent home, where she has been sexually abused by her father (Jayston). Both girls stay at a hostel for runaways. Debbie manages to get herself into college, while Sarah unsuccessfully tries to get her father prosecuted, but is instead returned home by the authorities. She runs away again, and ends up on the streets selling herself. She attempts suicide by stepping out in front of a lorry, but is injured and ends up in hospital. The film ends as she lies in hospital, while in the background a radio plays. Debbie calls a request show asking for a dedication for Sarah, with whom she has lost touch.
