Diana (Jemma Redgrave) is a high class young woman with a troubled past, who is on the verge of marrying the caddish Oliver (Mark Greenstreet). Spending the night in her old and spooky family mansion, Diana stares into a mirror and begins experiencing terrifying dreams. She sees her past, present and future, while at the same time entering a dark dimension of her own. One day Diana is visited by her friend Jenny (Kathleen Wilhoite), an American tourist and also a victim of a turbulent past. The death of King Henry VIII of England throws his kingdom into chaos as his heir, Edward VI, is both under-age and in poor health. Anticipating the young king's imminent death from consumption and anxious to keep England true to the Reformation by keeping the Catholic Mary from the throne, John Dudley, Lord President of the Council and second only to the king in power, hatches a plan to marry his son, Lord Guildford, to Lady Jane Grey, and have the royal physician keep the young king Edward VI alive—albeit in excruciating pain—long enough to get him to name Jane his heir.

Mr. Neville (Anthony Higgins), a young and arrogant artist and something of a Byronic hero, is contracted to produce a series of 12 landscape drawings of an estate, by Mrs. Virginia Herbert (Janet Suzman) for her absent and estranged husband. Part of the contract is that Mrs. Herbert agrees "to meet Mr. Neville in private and to comply with his requests concerning his pleasure with me." Several sexual encounters between them follow, each of them acted in such a way as to emphasise reluctance or distress on the part of Mrs Herbert and sexual aggression or insensitivity on the part of Mr Neville. Continuing from The Rocky Horror Picture Show are the characters of Brad and Janet Majors (now portrayed by Cliff De Young and Jessica Harper, respectively), now married. The film takes place in the town of Denton, USA, which has been taken over by fast food magnate Farley Flavors (also De Young). The town of Denton is entirely encased within a television studio for network DTV (Denton Television). Residents are either stars and regulars on a show, cast and crew, or audience members. Brad and Janet, while seated in the audience are chosen to participate in the game show Marriage Maze by the supposedly blind and kooky host Bert Schnick (Barry Humphries). As a "prize", Brad is imprisoned on Dentonvale, a soap opera that centers upon the local mental hospital run by brother and sister Cosmo and Nation McKinley (Richard O'Brien and Patricia Quinn).

A criminologist narrates the tale of the newly engaged couple Brad Majors and Janet Weiss who find themselves lost and with a flat tire on a cold and rainy late November evening. Seeking a telephone, the couple walk to a nearby castle where they discover a group of strange and outlandish people who are holding an Annual Transylvanian Convention. They are soon swept into the world of Dr. Frank N. Furter, a self-proclaimed "sweet transvestite from Transsexual, Transylvania". The ensemble of convention attendees also includes servants Riff Raff, his sister Magenta, and a groupie named Columbia.
