A year into his sentence, former commando John Rambo is visited by his old commander, Colonel Sam Trautman. With the war in Vietnam over, the public has become increasingly concerned over news that a small group of American POWs have been left in enemy custody. To placate their demands for action, the US government has authorized a solo infiltration mission to confirm the reports. He explains that he is putting together a mercenary team for a CIA-sponsored supply mission on behalf of anti-Soviet fighters in Afghanistan. Adonis Johnson Creed grew up without knowing his father Apollo, the one-time world heavyweight champion, and struggles to channel the strength, skill and toughness he inherited from him. After a fight in prison, he receives a visit from Rocky Balboa. Rocky had been given a world title shot by Apollo when he was just a club fighter in Philadelphia (Rocky), had eventually won Apollo's title in a rematch (Rocky II), had befriended him and been coached by him, rediscovering what Apollo called his "eye of the tiger" (Rocky III) and had been in Apollo's corner when Apollo had died in the ring against Ivan Drago (Rocky IV). Rocky shows Adonis who Apollo was and shows he believes Adonis can be a great like his father. In Upstate New York, a waste management firm loads barrels of toxic waste onto trucks, intending to illegally dispose of them. They are shown heading into a tunnel under the Hudson River between Lower Manhattan and Jersey City, New Jersey along with several other commuters, including struggling playwright Maddy Thompson (Amy Brenneman), some young offenders, a vacationing family, an elderly couple, and a mountain climber named Roy Nord (Viggo Mortensen). Meanwhile, a gang of diamond thieves in a stolen car try to escape the NYPD by racing into the tunnel. The gang force their way through the north tube traffic, spilling the diamonds all over the floor of the car. While a female thief tries to retrieve them, she pushes the gas pedal to the floor, causing the driver to lose control, smashing though a security booth and into one of the trucks, killing them instantly. The waste barrels explode, causing the other trucks to explode and setting off a chain reaction. The tunnel entrances cave in, and a devastating fireball sweeps through the tunnel, incinerating thousands of cars and people.

Adonis tracks down Rocky at Adrian's Restaurant - the Italian restaurant in Philadelphia that Rocky owns and operates, named after his late wife - and asks Rocky to train him, and although Rocky is reluctant to re-enter the sport of boxing, having already suffered from trauma to the brain (Rocky V) and made a one-off comeback at a very advanced age (Rocky Balboa) before re-entering retirement, Rocky agrees.

The setting for the story is an ant colony in Central Park in New York City. In 1984, Captain Ray Quick and Colonel Ned Trent, explosives experts working for the CIA, are on a mission to blow up a car transporting a South American drug dealer. In Upstate New York, a waste management firm loads barrels of toxic waste onto trucks, intending to illegally dispose of them. They are shown heading into a tunnel under the Hudson River between Lower Manhattan and Jersey City, New Jersey along with several other commuters, including struggling playwright Maddy Thompson (Amy Brenneman), some young offenders, a vacationing family, an elderly couple, and a mountain climber named Roy Nord (Viggo Mortensen). Meanwhile, a gang of diamond thieves in a stolen car try to escape the NYPD by racing into the tunnel. The gang force their way through the north tube traffic, spilling the diamonds all over the floor of the car. While a female thief tries to retrieve them, she pushes the gas pedal to the floor, causing the driver to lose control, smashing though a security booth and into one of the trucks, killing them instantly. The waste barrels explode, causing the other trucks to explode and setting off a chain reaction. The tunnel entrances cave in, and a devastating fireball sweeps through the tunnel, incinerating thousands of cars and people.
