On the eve of the French Revolution, Lucie Manette (Elizabeth Allan) is informed that her father (Henry B. Set in 1761 in London, the film focuses on events at St. Mary's of Bethlehem Asylum, a fictionalized version of Bethlem Royal Hospital, also known as "Bedlam." After an acquaintance of aristocrat Lord Mortimer dies in an attempt to escape from the asylum, apothecary general Master George Sims (played by Karloff, a fictionalized version of an infamous head physician at Bethlem, John Monro) appeases Mortimer by having his "loonies" put on a show for him. Mortified by the treatment of the patients, Mortimer's protégé Nell Bowen (Lee) seeks the help of Whig politician John Wilkes to reform the asylum. Mortimer and Sims conspire to commit Nell to the asylum, where her initial fears of the fellow inmates do not sway her sympathetic commitment to improving their conditions. Frustrated by Nell's progress with the inmates, Sims threatens her with his strongest "cure" but his attempt is thwarted by the very inmates that Nell helped. Ultimately, Sims is literally "deposed" and Nell is rescued by her Quaker friend who had counselled her through the whole process.

Gambler Babe Steward (Clark Gable) is in trouble with the law and decides to lie low in a small town. There he meets librarian Connie Randall (Carole Lombard) and attempts to seduce her. They flip a coin to decide whether or not to get married. The coin forces them to get married and Connie soon falls in love with Babe. Babe, meanwhile, continues his conning while telling Connie that he is working on Wall Street. Connie does not suspect anything until she finds Babe's marked cards in his desk. She shuffles the cards and when Babe plays a game of poker, he loses. Babe wants nothing more to do with Connie and leaves for Rio de Janeiro to win big money at cards. But, realizing that he loves Connie, he gives himself in to the police to serve his jail sentence. When Babe returns to a pregnant Connie, he does not suspect that she knows of his deception, but she does not say a word about it and in true Hollywood fashion, we are left to assume that the couple lives happily ever after.

An onscreen text warns of the superstitious belief in a vorvolaka, a malevolent force in human form. The film proper begins during the Balkan Wars of 1912. While his troops are burying their dead, General Pherides (Karloff) and American reporter Oliver Davis (Marc Cramer) visit the Isle of the Dead to pay their respects to the General's long-dead wife. They discover the crypt despoiled; hearing a woman singing on the supposedly uninhabited island, they set out to find her. They also find retired Swiss archeologist Dr. Aubrecht (Jason Robards, Sr.), his Greek housekeeper Madame Kyra (Helen Thimig), British diplomat Mr. St. Aubyn (Alan Napier) and his pale and sickly wife (Katherine Emery), her youthful Greek companion Thea (Ellen Drew), and an English tinsmith.

After the death of his wife Irena (Simone Simon), Oliver Reed (Kent Smith) has married former co-worker Alice Moore (Jane Randolph), and they now have a six-year-old introverted daughter, Amy (Ann Carter). Amy has trouble at school because she spends too much time daydreaming, no matter how much Oliver tries to encourage her to make friends and cope with reality. After Amy finds a photo of the deceased Irena, whose name is never mentioned in the house, Irena appears to her and the two strike up a friendship. At the same time, Amy befriends Julia Farren, an aging actress who is alienated from her own daughter Barbara (Elizabeth Russell), whom she suspects to be a "spy" only pretending to be her relative. Oliver, angry at Amy for repeatedly speaking of her new imaginary friend, punishes her. When Irena announces to Amy that she must leave her, Amy runs out of the house. A snow storm comes up, and Amy seeks shelter in the Farrens' house. Barbara, mad and jealous at her mother's preference for Amy over her, intends to strangle the girl. At this moment, Amy sees Irena's features in Barbara and embraces her. Barbara, perplexed by this gesture of affection, spares her life. In occupied France during the Franco-Prussian War of 1870, a beautiful young laundress, Elizabeth Rousset (Simone Simon), shares a stage coach ride from Rouen with a group of condescending nobles and businessmen and their wives, a political firebrand named Jean Cornudet (John Emery) and a young priest on his way to his new assignment (Edmund Glover). When they stop for the night at a village controlled by Prussian Lieutenant von Eyrick, known to his fellow officers as "Mademoiselle Fifi" (Kurt Kreuger), their coach is held up until the laundress agrees to "dine" with the lieutenant. Unlike her social betters, who have all fraternized with the enemy, and had them as guests in their homes, Elizabeth is a simple patriot, and will not eat or consort with the invaders of her country, so the coach cannot go on. The group finally convinces her that it would be best for France for them to get on with their business, and she concedes. While she is closeted with the arrogant Prussian, whose aim is to humiliate and degrade her, the rest of the travellers celebrate their deliverance by getting drunk on champagne, and following the progress of the evening's encounter through the sounds coming from inside.
