Fénelon, a Jewish singer-pianist, is sent with other prisoners to the Auschwitz concentration camp in a crowded train during World War II. After having their belongings and clothes taken and their heads shaved, the prisoners are processed and enter the camp. Fénelon is recognized as being a famous musician and she finds that she will be able to avoid hard manual labor and survive longer by becoming a member of the prison's female orchestra, Women's Orchestra of Auschwitz.

Gisella Perl (Christine Lahti), a Jewish-Romanian gynecologist from Sighetu, Romania, testifies before an Immigration & Naturalization Service (INS) review board consisting of three men (Bruce Davison, Richard Crenna, and Beau Bridges). Perl is seeking to be granted citizenship after passing the New York State Medical Licensing Board examinations, wishing to begin practicing in New York. She recounts her early life when she aspired to be a doctor despite the admonishments of her father, her time practicing as a gynecologist before the German invasion, and her experiences as prisoner #25404, where she provided what medical care she could to fellow prisoners. Her most controversial actions included providing late-term abortions to pregnant women in order to save their lives. These pregnant women would otherwise have been killed immediately or subjected to the torture of horrific "medical" experiments.

The film consists of five main sections: The Creation, Adam and Eve, Cain and Abel, Noah's Ark, and the story of Abraham. There are also a pair of shorter sections, one recounting the building of the Tower of Babel, and the other the destruction of Sodom and Gomorrah. The sections vary greatly in tone. The story of Abraham is somber and reverential, while that of Noah repeatedly focuses on his love of all animals—herbivorous and carnivorous or omnivorous. Cats (including lions) drink milk, with Noah's relationship with the animals being depicted harmoniously. It was originally conceived as the first in a series of films retelling the entire Old Testament, but these sequels were never made.

The film is set in 90 AD and concerns Jesus Christ's last surviving disciple, John of Patmos, and his writings and visions from the Apocalypse of John. Emperor Domitian has declared himself to be God and ruler over heaven and earth. The Christians do not recognize his divinity and face persecution from the Emperor. John has been imprisoned on Patmos, a mining prison, with many others. He writes out messages of his visions and sends them to the church in Asia. They send Irene to make sure John is alive and to confer with him in person. The movie depicts scenes from the Book of Revelation and links them to events happening to the church in Asia.

The film begins with a man whispering in despair, "The feeling begins. Very tender, very loving. Then the pain starts. Claws slip underneath the skin and tear their way up.
