Ramirez (Jacob Vargas) speaks in a voice-over about stories that his mother told him about the devil sometimes actively seeking out individuals who have sinned, while they're still alive on Earth. While taking human form, he traps them in a confined place only to turn them against each other, before killing them one at a time. Ramirez explains that the last victim will die in front of his or her loved one to make cynics of them all. He says that the signs are set in motion with a suicide, as that is when the Devil first makes his presence known. This story is abruptly interrupted by a violent suicide of a man who had fallen from the thirty-fifth floor of a Philadelphia office building. The truck he falls on rolls silently away.

Paul and Jessie Duncan (Greg Kinnear and Rebecca Romijn) are a happily married couple who have an eight-year-old son named Adam (Cameron Bright). The day after his eighth birthday, when fetching a basketball he was given into the street, Adam is killed in a collision. While leaving a church, Jessie and Paul are confronted by Dr. Richard Wells (Robert De Niro), an old professor of Jessie's. He offers to clone Adam, an illegal procedure which would require a change of location and identity, to which the Duncans reluctantly agree. Everything appears to be fine with the new Adam until he reaches his eighth birthday. That night, he experiences a violent nightmare. Richard explains to Paul that it is typical for boys his age to have night terrors, and that it is not serious. He explains that because Adam II has reached the age at which the original Adam died, his life cannot be predicted anymore. From that moment on, Adam II continues to have night terrors until they become visions and he starts having them when he's wide awake, losing control of his actions.

Perl is seeking to be granted citizenship after passing the New York State Medical Licensing Board examinations, wishing to begin practicing in New York. She recounts her early life when she aspired to be a doctor despite the admonishments of her father, her time practicing as a gynecologist before the German invasion, and her experiences as prisoner #25404, where she provided what medical care she could to fellow prisoners. Her most controversial actions included providing late-term abortions to pregnant women in order to save their lives. Two children, Ian and Clare, move into a town where their grandfather has just died. Ian thinks his grandfather's spirit is trying to tell them something, finding some library news clippings about a man named Zachariah Kull, who was accused of burning his home with his wife in it. Later, Ian and Clare capture a ghost named Jumper (a dead skydiver) who ends up freed by his partner, Coffin Ed (a revolutionary soldier). They follow them and other ghostly wisps of air into the forest, where they find an abandoned building. Entering, they find arriving ghosts entering a mirror, which leads to the afterlife. Ed and Jumper's boss, Mariah (a dead bride) isn't too pleased with this. Ian and Clare learn that they look for souls that don't want to cross over, and are dubbed the "Soul Patrol" by Jumper; they ended up with these jobs while waiting for their turn to cross over.
