The film opens with a man, later revealed to be named BJ, in a morgue who has sex with a corpse that has a biohazard symbol on the toe tag; afterward, he handles an empty test tube while washing up. Later, at a party, Samantha unsuccessfully attempts to contact her ex-girlfriend Nikki and meets with her friend Alice, who encourages Samantha to drink to forget about her recent break-up. Riley, who has an unrequited crush on Samantha, and Zain attempt to flirt with her, but Samantha brushes them off and excuses herself. She eventually gives in to Alice's intense pressure and becomes drunk. BJ introduces himself to Samantha and hands her a drink that he claims she dropped. Samantha, too drunk to know whether it is her drink or not, accepts it. Although she says she is a lesbian who is already involved in a relationship, as a result from being drunk and drugged, she agrees to have intercourse with BJ. From inside his car, Samantha begs BJ to stop and eventually blacks out.

Mangus Spedgewick is a typical high schooler living with his war veteran father, Mangus Sr., and neglectful stepmother, Raquel (Deborah Theaker), in a little town called River City, in Texas. It is a family tradition for the young male Spedgewicks to portray Jesus in the town production of Jesus Christ Spectacular, and Mangus is very excited about the upcoming auditions. Mangus lands the role, but that night, when he goes out in a limo with his friends, Timmy and Kimmy, there is a terrible car accident. Timmy and Kimmy are cut in half and Mangus loses the use of his legs.

The film begins with discussion amidst Hole's 1994 and 1995 world tour, then works backwards to Schemel's childhood growing up in Marysville, Washington, and details her coming out to her family as a lesbian, as well as her immersion in Seattle's music scenes, where she would eventually cross paths with Kurt Cobain. Through contemporary interviews with Schemel's bandmates Courtney Love, Eric Erlandson, and Melissa Auf der Maur, her beginnings in Hole are detailed, including her audition with Love and Erlandson in Los Angeles amidst the Rodney King riots, as well as her time living with Love and husband Kurt Cobain, and the songwriting process between Love, Schemel, and Erlandson. Additional commentary from fellow female drummers, musicians, peers, and friends of Schemel's are provided throughout. After the death of Hole's bassist Kristen Pfaff in 1994 (only two months after the suicide of Kurt Cobain), the band embarked on a world tour with Auf der Maur as Pfaff's replacement, and Schemel, along with Love, began heavily using heroin. Schemel's drug use leads to a breakup with her girlfriend, who acted as Love's personal assistant on the tour, and Schemel reflects on her time in a rehabilitation facility she checked into with Love after the conclusion of the tour in 1995.

Alike is a 17-year-old African American girl who hangs out at clubs with her openly lesbian friend Laura.
