In 1965, a couple (Sylvia Chang and James Wong) are doting on their newborn twin boys. Meanwhile, a dangerous gang leader named Crazy Kung (Kirk Wong) is being transported as a captive in the same hospital. Crazy Kung escapes and attempts to take one of the twins hostage. In the ensuing chaos, the twins are permanently separated. One of them, named Ma Yau, is raised by his parents and grows up to be a concert pianist and conductor. The other, named Wan Ma, is raised by a woman named Tsui (Mabel Cheung), and grows up to be a street racer and martial artist. Neither of them has ever thought that he has a twin brother.

The plot mostly involves the two main characters, Ching and Yiu and problems they face with Mickey, a fellow inmate and "Scarface", a high-ranking officer. Yiu is sentenced to three years for manslaughter. In a fight for his father, he kicked a man onto the path of a bus. Ching was sent to prison for killing his wife for being a prostitute. During the end of the film, inmates decide to go on hunger strike to protest against the cigarette price rise, this causes problems with "Assassin". Ching bit "Assassin"'s ear off and gets an extended sentence, Yiu is released from the prison.

Twelve years ago, Don (Louis Koo) was imprisoned due to murder and the fact he did not testify against the mastermind Dante (Simon Yam) and his friend Max (Julian Cheung), who was also involved of committing the crime. Twelve years later, Don is released from prison and decides not to repeat the same mistakes and make a fresh start. However, Don receives a sudden call from Dante and Max, whom force him to assassinate the candidate running for the president of the Philippines. Don rejects their request, but later discovers that the target have been assassinated. Don realizes he have been framed by Dante and begins to exile. Later, with the help of King Tso (Ray Lui), a legion soldier who was hired to take down the murderer, and Annie (Ada Choi), a female reporter, Don reveals the evidence of Dante's crime and cleared himself.

Police officer Pitt's (Wan Yeung-ming) partner Lau was killed by the triads. The case also involves the seizure of a number of pornographic photos of politicians from an earlier investigation. The murder was witnessed by chef Fat Goose (Sammo Hung), and Pitt forces him to be the eyewitness and have the killer arrested. Warsaw's liberation allies is actually a terrorist organization under the banner of a revolution and in order to show their strength, they prepare to create a bombing incident when the US business delegates visit Hong Kong to achieve their political objectives. The CIA finds clues and sends Detective Gary Redner (Peter Lapis) to Hong Kong to prevent this conspiracy. In Cambodia 1975, the eight-year-old Wai Lok-yan witnessed the tragedy of his parents being killed by Ray Lui (Paul Chun) while hiding in the closet with his younger sister. Back then, Yan's father Wah (Phillip Ko), Ray and Seung (David Chiang) were war comrades working for the CIA in Cambodia. Like the first film, this follows the fraternal bond of two inmates, once again Ching, but this time with "Brother Dragon", a boss of the "Black Rats" gang. Ching escapes the prison in order to see his son, Leung was placed in an orphanage after his grandmother died, and gets into trouble with "Assassin"'s replacement, Officer Zau. Ching escapes a second time to follow Boss Dragon after he lied about jumping off a cliff to escape. Ching looked for his son and intended to bring him along, but was refused by the social worker.
