The Invisible War features interviews with veterans from multiple branches of the United States Armed Forces who recount the events surrounding their assaults. Their stories show many common themes, such as the lack of recourse to an impartial justice system, reprisals against survivors instead of against perpetrators, the absence of adequate emotional and physical care for survivors, the unhindered advancement of perpetrators' careers, and the forced expulsion of survivors from service.

The title comes from the Swahili term "maafa," which means tragedy or disaster and is used to describe the centuries of global oppression of African people during slavery, apartheid and colonial rule, while the number "21" refers to an alleged maafa in the 21st century (though beginning in the 19th), which the film says is the disproportionately high rate of abortion among African Americans. The film states that abortion has reduced the black population in the United States by 25 percent. It discusses some of Planned Parenthood's origins (formerly the American Birth Control League), attributing to it a "150-year-old goal of exterminating the black population." It attacks Margaret Sanger, along with other birth control advocates, as a racist eugenicist. The film features conservative African Americans who are associated with the Tea Party movement, including politician Stephen Broden, and Martin Luther King, Jr.'s niece Alveda King, who claims that Sanger targeted black people.

Outrage argues that several American political figures have led closeted gay lives while supporting and endorsing legislation that is harmful to the gay community. The film examines mass media's reluctance to discuss issues involving gay politicians despite many comparable news stories about heterosexual politicians and scandals. Outrage describes this behavior as a form of institutionalized homophobia that has resulted in a tacit policy of self-censorship when reporting on these issues. The film is based on the work of blogger Michael Rogers and his site BlogActive.com.

Saving Face features two women attacked by acid and their struggle for justice and healing. It follows London-based Pakistani plastic surgeon Dr. Mohammad Jawad as he journeys to Pakistan to perform reconstructive surgery on survivors of acid violence. Saving Face broaches the subject of the under-reporting of acid violence against women due to cultural and structural inequalities towards women from Pakistani men. The Acid Survivors Foundation of Pakistan, which is featured in the film, had documented more than 100 acid attacks a year in Pakistan but estimates far more due to lack of reporting.
