Synopsis
The morbidly obese Ethel Janowski (institutionalized due to her bouts of paranoia, depression, and violence) is released into the care of her grandmother, despite Doctor Gerard's unease regarding Ethel's discharge. Upon moving into her grandmother's San Francisco home, Ethel begins consuming massive amounts of food, and repeatedly claims that the employees of the institution were trying to starve her to death. In an attempt to stop Ethel's gorging, Mrs. Janowski empties the refrigerator, and locks the cupboards. Ethel and her grandmother argue, and when the elder Janowski threatens to call the sanitarium, Ethel impales her with a knife, then mutilates the old woman's hand to get the cabinet key she was holding in a death grip.Actors

A couple go to a drive-in theater in a rural California town, and are butchered by an unseen assailant, who uses a sword to decapitate the man, and skewer the woman through the neck. Investigating this dual homicide are police detectives Mike Leary and John Koch, who interview the drive-in's boorish manager, Austin Johnson, and the odd custodian, Germy. Germy mentions that a peeping tom likes to cruise the area to watch couples and lone girls, and he is told to try and write down the voyeur's license plate number the next time he sees him.

May Dove Canady is an awkward, lonely young woman, who suffered a troubled childhood due to her lazy eye. She has very few social interactions, her only "true friend" being a glass-encased doll named Suzie made by her mother and given to May for her birthday with the adage "If you can't find a friend, make one." May works at a veterinary hospital, assisting with surgeries. Her optometrist fixes May's lazy eye, first with glasses, then with a special form of contact lens. As May attempts to be more social, she becomes friends with Adam, a local mechanic. She has a fixation on his hands, which she considers to be the most attractive part of him, and they start dating. Meanwhile, May's lesbian colleague Polly begins to flirt with May. One day while feeling especially low, May remarks that Polly has a beautiful neck. They become friends and Polly gives her pet cat Lupe to May, ostensibly because of her "bitch" landlord.

Los Angeles is being terrorized by a masked serial killer whose modus operandi is to break into stores after hours, murder the workers, and take the surveillance tapes so he can use them to relive his crimes. At Steve's liquor store (the main door to which has been boarded up due to an incident with a homeless man) the employees, customers, and a deliveryman named Morty discuss the killer, the $500,000 being offered as a reward for his capture, and how they would kill him, the mentioned methods being eye gouging, exsanguination in plastic wrap, sawing, and decapitation. After the store closes, Steve has a clerk named Donny, and two regulars (a prostitute named Jess, and a phone sex worker named Mona) stay to play cards in the upstairs apartment as a means of keeping Steve's nephew Jimmy (an addict going through drug withdrawal) occupied. Unbeknownst to the quintet, the Convenience Store Killer has chosen them as his next victims, cutting the homeless man (who resides in a nearby alley) in half before sneaking into the building.

Paul and Mary Bland are a wine dealer and a nurse, respectively, who bemoan their low status in life and dream of opening a restaurant. Ilsa is Kommandant of a Nazi prison camp, who conducts sadistic scientific experiments designed to demonstrate that women are more capable of enduring pain than men are, and therefore should be allowed to fight in German armed forces (it is late in the war and the Nazi military is in dire need of reinforcements). Ilsa is also portrayed as a buxom woman with a voracious sexual appetite for men. Every night, she chooses another one of her male prisoners and rapes him; however, due to her insatiable hunger, she gets disappointed when her current victim eventually ejaculates, and promptly has him castrated and put to death. Only an American prisoner, who can withhold ejaculating, manages to use her weakness to his favor. He hopes that the Allies will arrive soon, but a faction of the SS wants to eliminate all evidence and witnesses.

The film follows Harry Russo, a psychotic, drug-addicted crossdresser and film writer who is obsessed with a puppet named Rubberneck, the mascot of his favorite film which his girlfriend, Drew, had given him. Harry believes Rubberneck speaks to him, instructing him to commit acts of violence, such as killing "whores" (or "who-ers" as Harry pronounces it). Egged on by Rubberneck, Harry fatally chokes two prostitutes (mutilating and having anal sex with their bodies afterward) and stabs his psychiatrist to death.
