Il est vaguement basé sur le 869e épisode de l'émission de télévision Unanswered qui raconte une histoire vraie s'étant passée à Pusan, où des meurtres n'ont jamais été rapportés, des corps jamais retrouvés et des enquêtes n'ayant jamais eu lieu. Les familles des victimes ont essayé d'interdire la sortie du film en portant l'affaire devant les tribunaux car il représentait avec exactitude et sans leur consentement les faits et les meurtres de leurs proches. Elles retirent finalement leurs plaintes le 1er octobre 2018 après avoir reçu les excuses solennelles de l'équipe de production.
Il totalise plus de 3,7 millions d'entrées au box-office sud-coréen de 2018 pour 28,3 millions $ de recettes.
L'expression dark figure of crime (la « face obscure du crime ») est un terme utilisé en anglais pour désigner les crimes non signalés ou non découverts.

Eom Joong-ho is a dishonest ex-detective turned pimp who is in financial trouble because two of his girls have gone missing. One night he sends Mi-jin, one of his few remaining girls, to a customer. But he later realizes that this is the same person who was the last to see his missing girls. Believing that this man is reselling his women, he goes to look for Mi-jin. Jae-hyuk (Kim Myung-min) is a former professor with a doctorate in biochemistry who is currently working as a pharmaceutical sales representative after losing his life savings and his job due to a bad investment he made in the stock market from advice he received from his younger brother. When a series of dead bodies are found floating in the Han River, the public is shocked to discover that the deaths are related to a fatal outbreak of virus-infected mutant parasitic horsehair worms, called Yeongasi, that can control the human brain. Those infected show symptoms of increased hunger without appropriate weight gain and excessive thirst when the worms are mature and ready to reproduce. Hence, they jump into the river to allow the worms to come out of the body. While the authorities work to find a cure, Jae-hyuk and his brother Jae-pil (Kim Dong-wan), a detective agonized with guilt for squandering Jae-hyuk's money in the stock market, struggle to save Jae-hyuk's family when they also exhibit similar symptoms.
