The film is set in an unspecified time on Earth when the descendants of a genetically enhanced race created by mankind have control of the planet. Known as the "ghen" - a derivative of the word "generic" which humans labeled them as—the genetically superior race rule from their underground utopia, Hollow Earth. Baron Crecilius Pryme, a charismatic political from Ume City, sets out to convince his fellow ghen that what remains of the human race must be eradicated because he believes they are forming a crusade to attack the ghen. Crecilius shrewdly moves his brother, Aiden Pryme, into his deadly group of private soldiers known as "The Ministers", who hunt and abuse humans above ground.

In 1944, the Nazis build a dimensional portal off the coast of Scotland. With the help of Russian mystic Grigori Rasputin, they intend to free the Ogdru Jahad—monstrous entities imprisoned in deep space—to aid them in defeating the Allies. Rasputin knows this will be an apocalyptic event that he believes will create a paradise. He opens the portal with the aid of his disciples, Ilsa von Haupstein and Obersturmbannführer Karl Ruprecht Kroenen, member of the Thule Society and Adolf Hitler's top assassin. An Allied team is sent to destroy the portal, guided by young Trevor Bruttenholm, who is well-versed in the occult. The German team is killed and the portal is destroyed—in the process absorbing Rasputin—while Haupstein and Kroenen escape. The Allied team discovers that an infant demon with a right hand of stone came through the portal; they dub him "Hellboy" and Bruttenholm adopts him.

During Christmas 1955, an 11-year-old Hellboy is told a bedtime story by his adoptive father, Trevor Broom, involving an ancient war between man and magical creatures, started by man's greed. After defeat of the magical creatures' forces, the master of the goblin blacksmiths offers to build an indestructible mechanical army for the elven King Balor. Encouraged by his son Prince Nuada, Balor orders the building of this Golden Army. The humans are devastated by the army. Balor is ridden with guilt and forms a truce with the humans: Man will keep his cities and the magical creatures will keep their forests. Nuada does not agree with the truce and leaves in exile. The magical crown controlling the army is broken into three pieces, one going to the humans and the other two kept by the elves.

Two hundred years after the events of Alien 3, military scientists on the outer space vessel USM Auriga create a clone of Ellen Ripley (Sigourney Weaver) using DNA from blood samples taken before her death. They extract the embryo of an Alien queen that had been growing inside her at the time of her death, raise it, and collect its eggs for further use. The Ripley clone is kept alive for further study. Adam Ewing, an American lawyer, has come to the Chatham Islands to conclude a business arrangement with Reverend Horrox and his father-in-law. While in Horrox's plantation, he witnesses the whipping of a Moriori slave, Autua, who later stows away on the ship. Autua confronts Ewing and convinces him to advocate for Autua to join the crew as a free man. Dr Henry Goose slowly poisons Ewing, claiming it to be the cure for a parasitic worm, to steal Ewing's valuables. As Goose administers the fatal dose, Autua saves Ewing. Returning to the United States, Ewing and his wife Tilda denounce her father's complicity in slavery and leave to join the abolition movement.
