A serial killer, having murdered over 30 people, is on the loose in a Los Angeles suburb. A television repairman with a pronounced limp, named Horace Pinker (Mitch Pileggi), becomes the prime suspect. When the investigating detective, Lt. Don Parker (Michael Murphy), gets too close, Pinker murders Parker's wife, foster daughter, and foster son. However, his other foster son Jonathan (Peter Berg) develops a strange connection to Pinker through his dreams and leads Parker to Pinker's rundown shop. In a shootout in which several officers are killed, Pinker manages to escape. He targets Jonathan's girlfriend Allison (Camille Cooper) in retribution.

During the filming of a death scene of The Death Kiss, leading man Myles Brent is really shot and killed. Tonart Studios manager Joseph Steiner (Lugosi) is assigned to handle the situation. The studio wants to pass it off as a simple accident, but screenwriter Franklyn Drew (Manners) digs a bullet out of a wall and tells Homicide Detective Lieutenant Sheehan that it is a .38 caliber, while the guns used in the film are all .45s.

Ben is a witty, charismatic serial killer who holds forth at length about whatever comes to mind, be it the "craft" of murder, the failings of architecture, his own poetry, or classical music, which he plays with his girlfriend. For the past 13 years, Dr. Herbert West (Jeffrey Combs) has been serving a prison sentence due to a murder at the hands of one of his zombies. With what scant supplies he has on hand in the prison medical center, Dr. West has been capable of performing only extremely basic experiments on rats. However, his lack of supplies does not prevent him from uncovering a key element in his re-animation process. Dr. West has discovered "NPE" (Nano-Plasmic Energy), an energy that can be extracted from the brain of a living organism through an electrocution-like process, to be stored in a capsule resembling a small light bulb. The capsule can then be connected to a corpse and used in conjunction with West's previously developed reagent to restore the former dead to a lifelike state. The NPE prevents the degeneration seen in previous instances, where the reanimated are nothing more than mindless zombies. Used together with the re-agent, reanimated corpses regain their skills, memories, and motor functions and nearly fully resemble normal humans.

As a boy, a reclusive and antisocial Sufferton resident, Max Seed, was disfigured in a school bus crash that killed everyone else involved in it. In 1973, Seed began torturing and murdering people, filming some of his victims starving to death in his locked basement, and ultimately racking up a bodycount of 666. In 1979, Seed is arrested by Detective Matt Bishop in a siege that claims the lives of five of Bishop's fellow officers. Seed is sentenced to death by electric chair, and incarcerated on an island prison, where he is a model inmate, only acting out when he kills three guards who try to rape him.

In 2012, private corporations own and manage the prison systems. Getaway driver Carl "Luke" Lucas (Luke Goss) is arrested after a robbery for his crime boss Markus Kane (Sean Bean) goes wrong. As his accomplices are robbing the bank, two officers casually enter the building. Luke tells them to abort, but they refuse; Luke intervenes, but it results in the death of one of the three accomplices. Luke shoots and kills one of the officers and dumps off his accomplices in order to fulfill Markus's wishes. In doing so, Luke is eventually captured by the police following a high-speed chase and sentenced to serve time on Terminal Island.
