Pendant une froide journée d'hiver, Sun-yeong, une jeune femme enceinte, effectue un long trajet en taxi vers sa ville natale où vit sa mère et où elle va accoucher. Un auto-stoppeur avec une casquette de baseball, étrange et impoli, partage le taxi avec elle et le chauffeur de taxi. Il prétend tout connaitre d'elle et du chauffeur de taxi, raconte des insanités et entame un étrange compte à rebours, suivi d’un flash, un bruit assourdissant, puis le néant… . Quand il arrive à zéro, le taxi s’arrête et ils perdent connaissance. À son réveil, Sun-yeong est seule, réalise qu’il n’y a plus d’électricité et que des grognements effrayants émanent des bois.

Kim Sun-yi, an elderly woman in her sixties living in the US, receives a phone call about the sale of her old family home back in South Korea. Returning to her homeland, she's met by granddaughter Eun-joo, and they drive to the house in the country and stay the night. Sun-yi recalls how 47 years ago when she was a teenage girl in 1965, she moved from Seoul along with her widowed mother and sister Sun-ja to a remote valley to undergo a period of convalescence after suffering problems with her lungs. The Kims lived in genteel poverty at the mercy of their arrogant and foppish landlord, Ji-tae, son of the business partner of Sun-yi's late father. Because of her delicate health, the beautiful yet introverted Sun-yi lives an isolated life in the country home, without any friends her age.

In 1988, businessman Oh Dae-su (Choi Min-sik) is arrested for drunken behavior, missing his daughter's 4th birthday. After his friend, Joo-hwan (Ji Dae-han), picks him up from the police station, they go to a phone booth to call home to let Dae-su's family know of his whereabouts. While Joo-hwan is talking to Dae-su's wife on the phone, Dae-su is kidnapped. He wakes up in a solitary confinement in a hotel-like prison. Confined with no human contact or explanation for his kidnapping and frequently gassed with a possibly mind altering drug, Dae-su soon learns through news reports his wife has been murdered, and he is the prime suspect. Dae-su passes the time shadowboxing, planning revenge, and secretly attempting to tunnel out of his cell.

An aspiring opera singer In-jeong travels to countryside with her college professor Yeong-seon. She wants to learn more about the outcome of her audition for a part in an upcoming opera performance. The two park on a deserted riverbank to make a campfire. Instead of talking about the audition, Yeong-seon tries to rape In-jeong who escapes to a forest. Yeong-seon wants to leave but his car gets stuck. Three local thugs with motorcycles discover the car and confront him. Meanwhile, In-jeong stops a man on a scooter who promises to take her to a public transport terminal after they meet his friends who are waiting at the river. As it turns out, he is the leader of the group and takes In-jeong back to the riverbank where she reunites with the scared Yeong-seon but the two do not admit knowing each other. The thugs eat roasted pork and have a small talk with the professor when a young student Hyeon-jae rolls out from a bag that was placed on one of the motorcycles. The thug leader starts harassing the boy. His behavior becomes more psychotic when In-jeong voices her disapproval. He orders the thugs to rape her and forces the student and Yeong-seon to fight until one of them drops. Hyeon-jae knows taekwondo and easily defeats Yeong-seon. He then proceeds to beat the thugs. With all of them unconscious, the boy digs a hole in the ground, buries the men up to their chests and pours gasoline on their heads preparing to set them on fire. Yeong-seon runs for help and In-jeong tries to stop the student. While she's talking to him, the leader regains consciousness and manages to hit the boy with a shovel and escape from the hole. After waking up, one of the thugs beats the injured boy so badly that all believe him dead.

After his best friend dies in an unexpected accident, black market dealer Young-gyu decides to wash his hands off his illegal activities and start a new life. He then falls in love with an acquaintance, Yu-Ri, who is unaware of his feelings and unknowing of his past. He later learns that Yu-ri needs money for her father's surgery and she has resorted to procuring the services of a local organ broker, who is Young-gyu's former client. The broker promises to help her secure a legal organ donation, but secretly relies on black market dealers like Young-gyu, who engage in multiple kidnappings and killings. To help her out, Young-gyu agrees to do the job one last time when he is once again approached by his former client. His smuggling ring gets back together for a final run and they resume their M.O. of operating out of a ferry boat that runs between Korea and China, picking victims from among its passengers, abducting them from their rooms, harvesting their organs on board the ship while they are still alive yet restrained, and then brutally disposing of the bodies afterwards.

Set in the 1930s during Japan's colonial rule of Korea, many Korean resistance activists are based in Manchuria, northeastern China while hiding from Japanese authorities. A Christian musical procession waits outside a prison for Lee Geum-ja (Lee Young-ae), a recently reformed female prisoner, who had been convicted of murdering a young schoolboy, Won-mo. Lee Yong-go is a mentally impaired father with the intellect of a six-year-old, who lives in a run-down house along with his daughter of the same age, Ye-sung. One day, he gets into a physical altercation with the police commissioner, who has just purchased the last Sailor Moon backpack for his daughter, a gift Yong-go was saving up to buy for Ye-sung. Soon after, the police commissioner's daughter dies in a freak accident, in which she slips on ice and suffers a fatal blow to the back of her head while she is taking Yong-go to another store that sells the same backpack. When he tries to resuscitate her, a woman witnesses him and mistakes him to be molesting her. Yong-go is falsely accused of the abduction, murder, and rape of a minor. The police quickly took advantage of his disability and forced him to admit committing the crimes while ignoring exonerating evidence. Yong-go, who is sentenced to death, is imprisoned and assigned to Cell No. 7, the harshest cell in a maximum security prison.
