In a filthy kitchen of an underground abortionist, an unkempt former medical doctor puts on surgical gloves and quickly downs a full glass of cheap whiskey. Face down on the kitchen table is a barely conscious Porter (Mel Gibson), severely wounded with two large bullet wounds in his back. The doctor pours whiskey on Porter's back to sterilize the area and digs out the bullets. Porter spends five months recuperating. The film opens with a getaway driver (Mel Gibson) and a wounded accomplice dressed as clowns fleeing from American police toward the Mexican border. The driver crashes through the border fence, whereupon he is arrested by corrupt Mexican police officers Vasquez (Mario Zaragoza) and Romero (Gerardo Teracena). When they find over $2 million in the car, they incarcerate him in El Pueblito prison under false charges, keep the cash for themselves, and cremate the accomplice, who by then has died. As one of the only Americans incarcerated there, the driver becomes known as "the Gringo". El Pueblito proves surprising, operating more like a small ghetto than a prison. The Gringo quickly manages to work out the prison's criminal hierarchy and engages in petty thefts and robberies from some of the prison's less reputable businesses. One of these thefts is witnessed by an unnamed kid (Kevin Hernandez) who is living with his incarcerated mother (Dolores Heredia) and is protected by the prison's criminals. Curious, the Gringo presses him to explain why the criminals protect him, but the Kid refuses.

The film starts with Machete Cortez (Danny Trejo) and Sartana Rivera (Jessica Alba) attempting to capture weapon dealers who have been supplying the Mexican drug cartels. The military men are then all killed by the gangsters, who in turn are wiped out by another intervening party. Its leader murders Sartana while Machete is arrested by corrupt Sheriff Doakes (William Sadler) and Deputy Clebourne (Samuel Davis). Doakes unsuccessfully tries to hang Machete but the President of the US, Rathcock (Charlie Sheen), intervenes. Machete is brought to the White House, where the president offers him US citizenship if he eliminates Marcos Mendez (Demián Bichir), a psychopath who is threatening to fire a nuclear missile at Washington, D.C. if the American government does not intervene to stop the rampant drug cartels in Mexico and the corruption of its government.

L.A.P.D. Sergeants Martin Riggs (Gibson) and Roger Murtaugh (Glover) arrive at an evacuated building believed to have a bomb in it. Against orders, they go inside to investigate the bomb. Riggs decides to deactivate the bomb himself instead of waiting for the Bomb Squad to arrive. Unfortunately, he causes the bomb to detonate and the whole building to collapse. This causes them to be demoted to uniform duties, which upsets Murtaugh because he only has seven days until retirement. While on street patrol, the two thwart a robbery using a duplicate armored car, although they only manage to catch one of the two would-be thieves. During the arrest, it is discovered the criminals were armed with a new type of armor-piercing bullet, informally referred to as "cop killers". For their efforts in stopping the raid, Riggs and Murtaugh are reinstated as Detective Sergeants by their superior, Captain Murphy (Kahan). Murphy introduces internal affairs Sergeant Lorna Cole (Russo) to the two detectives and informs them that the suspect they have in custody from the raid is a known associate of former L.A.P.D. Lieutenant and suspected arms dealer: Jack Travis (Wilson).
