In Mexico in the 17th century the Viceroy of New Spain and the newly elected Archbishop discuss how they will rule. The Archbishop sends a letter to the convent where Sor Juana lives informing the abbess that his first official order of business will be to visit their convent and to meet Sor Juana. For the occasion Sor Juana presents a play she has been working on. The Viceroy and his wife, the Vicereine are impressed with Sor Juana's writing. The Archbishop on the other hand is disturbed that the convent is so lax. He secretly meets with a few nuns and persuades them to elect a more formal abbess.

Diego Montes (Nacho Martínez) is a bullfighter, forced into early retirement by a goring, and in the opening scene he finds sexual gratification viewing videos of violent death. In retirement he teaches bullfighting, and among his students is Ángel, a diffident young man who suffers from vertigo. During one episode of vertigo in the practice ring, the scene cuts from Ángel to a woman killing a man during sex with a pin she was wearing in her hair, striking at the base of the neck between the shoulder blades, as a matador kills a bull. After class, Diego asks Ángel if he is homosexual, noting that he is not experienced with women. Ángel says he is not and vows to prove himself. Later that day, Ángel attempts to rape Eva (Eva Cobo), a model who is both his neighbour and Diego's girlfriend. As she is leaving him, she trips in the mud and gashes her cheek. At the sight of her blood, Ángel faints.

Carmilla arrives in South Carolina to investigate a woman named Millarca. The only information she has is an address, but Troy, the owner of the house, says nobody named Millarca has ever lived there. He offers to give Carmilla a ride into town, and she later coincidentally visits a restaurant where Troy's daughter, Laura, works. Curious, Laura presses for more details, and Carmilla reveals that Millarca is her mother, whom she never knew. Carmilla eventually becomes annoyed with Laura's questions and leaves, but Laura stops her to explain that her father once rented out a trailer. Carmilla requests public records at the police station, but they tell her it will take two business days.

It is New Year's Day of 2006, more than a year after the events of Night Watch. During the summer in Brighton Beach, Brooklyn, elderly widow Sara Goldfarb (Ellen Burstyn) constantly watches television, particularly infomercials hosted by Tappy Tibbons (Christopher McDonald). After receiving an unexpected phone call that she has won a spot to participate on a television game show, she becomes obsessed with regaining the youthful appearance she possesses in an old photograph from her son Harry's (Jared Leto) graduation many years earlier. In order to fit into her old red dress seen in the picture, the favorite one of her deceased husband Seymour, she goes on a crash diet. In order to reach her goal sooner, she goes to a doctor to discuss weight loss. The doctor gives her a prescription for weight-loss amphetamine pills throughout the day and a sedative at night. Harry warns her about amphetamine dependence and risk of life-threatening consequences, but she rebuffs him and insists that the chance to be on television has given her a reason to live. As the months go by, Sara's tolerance for the pills adjust and as a result she is no longer able to feel the same high the pills once gave her. When her invitation has still not arrived, she wrongfully increases her dosage from double to triple and, as a result, begins to suffer from amphetamine psychosis. Soon, her delusions worsen and she is driven to the brink of madness when she suffers a hallucination that she appears on the game show as the principal subject while being attacked by her monstrous, anthropomorphized refrigerator.

Alone, that is, until the sun sets and a mysterious Hungarian countess, Elizabeth Báthory (Delphine Seyrig) arrives in a vintage Bristol driven by her "secretary" Ilona (Andrea Rau). The middle-aged concierge at the hotel swears that he saw the Countess at the same hotel when he was a little boy. The pair may have a connection to three separate gruesome murders of young girls that occurred in Bruges the previous week. On a day trip, Stefan and Valerie witness the aftermath of a fourth. At the hotel, the countess quickly becomes obsessed with the newlyweds, and the resulting interaction of the four people leads to sadism and murder. Ilona, Stefan, then the Countess all die, leaving Valerie, now transformed into a creature similar to the Countess, stalking new victims.
