The film recounts the last few days in the life of Elvira (Volker Spengler), a transsexual woman formerly known as Erwin. After being beat up for trying to buy sex in a park, she returns home to her longtime lover Christoph (Karl Scheydt), who's been away for six weeks. Christoph abuses her verbally and physically, and when he announces he's leaving for good, she desperately tries to stop him, only to be rescued by her friend Zora (Ingrid Caven). We watch as Elvira and Zora visit a slaughterhouse and the orphanage where Elvira grew up, and her later contacts with her family and former lover Anton Saitz (Gottfried John), as she attempts to come to terms with the consequences of her decision to change her sex.

During World War II the British army is attempting to retrieve an Enigma machine from Germany. Having failed in previous attempts they decide to send four men undercover to the factory that makes the devices, deep in Berlin. Unfortunately the factory is populated entirely by women, and they only have men to send. American O'Rourke (LeBlanc), British transvestite Tony Parker (Izzard), genius Johnno (David Birkin) and the reluctant Archie (James Cosmo) are sent to infiltrate the factory dressed as women.

Petra von Kant (Carstersen) is a prominent fashion designer based in Bremen. The film is almost totally restricted to her apartment's bedroom, decorated by a huge reproduction of Poussin's Midas and Bacchus (c.1630), which depicts naked and partially clothed men. German tourist Jasmin Münchgstettner (Sägebrecht) from Rosenheim and her husband fight whilst they are driving across the desert. She storms out of the car and makes her way to the isolated truck stop, which is run by the tough-as-nails and short-tempered Brenda (Pounder), whose own husband, after an argument out front, is soon to leave as well. Traude Krueger (Bleibtreu) is working as a piano teacher in a women's prison. While selecting new students, she meets Jenny Von Loeben (Herzsprung). When she tells her she can't take any lessons because her hands are too rough and she is uncooperative, Jenny becomes enraged and almost kills the prison guard, Mütze (Pippig), also one of Krueger's students. Then she starts playing the piano. Krueger listens from the hallway and, impressed by her talent, later offers Jenny lessons, but requires absolute obedience, including eating a sheet of paper. She tells Jenny never to play 'that kind of negro-music' again.

The film opens with a montage of images of Manhattan and other parts of New York City accompanied by George Gershwin's Rhapsody in Blue, with Isaac Davis (Woody Allen) narrating drafts of an introduction to a book about a man who loves the city. Isaac is a twice-divorced, 42-year-old television comedy writer dealing with the women in his life who quits his unfulfilling job. He is dating Tracy (Mariel Hemingway), a 17-year-old girl attending the Dalton School. His best friend, college professor Yale Pollack (Michael Murphy), married to Emily (Anne Byrne), is having an affair with Mary Wilkie (Diane Keaton). Mary's ex-husband and former teacher, Jeremiah (Wallace Shawn), also appears. Isaac's ex-wife Jill Davis (Meryl Streep) is writing a confessional book about their marriage. Jill has also since come out of the closet as a lesbian and lives with her partner, Connie (Karen Ludwig).
