The young emperor Nero proves himself spoiled, childish and unable to cope with the government of Rome. A group of scientists are working on a serum called Death One, which reanimates the dead. When Dr. Alan Holder (Robert Marius) and his assistant Norma experiment on a human corpse, the corpse becomes a zombie, prompting Dr. Holder to resign from the project. As he prepares to surrender the serum to waiting military officers, a small group of criminals ambush the center; all but one are killed, and the surviving criminal manages to abscond with Death One. During a chase, the container with the serum is shot out of his hand and breaks; as he tries to pick it back up, he accidentally touches the serum. He flees to the Sweet River Resort and settles in Room 4. General Morton (Mike Monty) promises Dr. Holder that he and his men will capture the criminal. By the time they catch up to him, however, he has already succumbed to the disease, but not before infecting a bellhop and killing a maid before finally cutting off his own hand in a failed attempt to stop the spread of the infection. General Morton orders the patrons and staff killed and buried in a mass grave, and condemns the resort; the criminal's remains are delivered to him and his right-hand men, Tracey (Bruno Mattei) and Cheney (Claudio Fragasso), and burned. While attending the funeral of his father, Melvin Devereux (John Savage), meets a beautiful young woman (Sandi Schultz) who addresses him by name, although he cannot remember having met her before. Giorgio Mainardi (Diulio Del Prete), a wealthy, middle-aged financier, lies dying in a hospital bed surrounded by his family. He has collapsed suddenly, coughing up blood. His last word is to his stone-faced family is, "why?" The doctor's diagnosis of Giorgio's death is an internal hemorrhage, possibly from a stomach or intestinal ulcer. The following day, Giorgio's daughter Rosie (Karina Huff) arrives at the Mainardi estate having taken a leave of absence from her college studies to attend the funeral and for the reading of the will. Rosie soon finds her entire family squabbling over the estate. Giorgio's stepmother Hilda (Frances Nacmen), refuses permission for an autopsy, and taunts her elderly husband, Giorgio's father Paolo (Paolo Paoloni), who is near death himself from a recent stroke and is unable to move or speak. Meanwhile, Hilda's son Mario (Pascal Persiano) is revealed to be having an affair with Giorgio's third wife Lucy (Bettina Giovannini). Lucy is only a few years older than Rosie, and is apparently the only one of the Mainardi family who shows any compassion for her presence.
